1/26. epidermal cyst of the clitoris: a rare cause of clitorimegaly.

    Epidermal cysts are slowly growing, intradermal or subcutaneous tumors with a wall composed of true epidermis. They occur most commonly on the face, scalp, neck, and trunk. We report the unusual case of a 16-year-old girl with an epidermal cyst of the clitoris. The tumor was removed by local excision.

2/26. Vulvar epithelial inclusion cyst as a late complication of childhood female traditional genital surgery.

    Epithelial inclusion cysts may be a late complication of traditional female genital surgery. We present the case of a 19-year-old Eritrean woman who had been circumcised at birth and sought treatment for an enlarging vulvar mass that arose after the onset of puberty. Symptoms included pain, pruritus, and interference with walking.

3/26. Operative laparoscopy and vulvar hematoma: an unusual association.

    Few cases of intraoperative or postoperative complications associated with laparoscopic adnexal surgery have been reported in the literature. We describe a case of laparoscopic abdominal vascular injury and persistent bleeding in the matrix of the ovary following laparoscopic cystectomy. During the first postsurgical day, the patient was syncopal. The physical examination showed a vulvar hematoma and minimal bleeding from a laparoscopic incision in the abdominal wall. Vulvar hematoma and an unstable patient may signal serious vascular bleeding.

4/26. Epidermal clitoral inclusion cyst after type I female genital mutilation.

    OBJECTIVE: To document the occurrence of long-term sequelae after type I female genital mutilation (FGM) and describe the surgical treatment of epidermal clitoral inclusion cyst. STUDY DESIGN: Twenty-one women presented with epidermal clitoral inclusion cyst after type I FGM at the Department of obstetrics and gynecology at King Fahad Armed Forces Hospital, Jeddah, saudi arabia. The duration (mean /- SD, range) of symptoms was 10.3 /- 5.4, 2 to 20 years. They were treated by excision of the cyst with particular attention to preserve the remaining part of the clitoris. The technique involves making a vertical incision in the skin, dissecting and excising the cyst, removing the excessive skin, and reapproximating the skin edges. RESULTS: The procedure was done on all patients without intraoperative complications. All except one were discharged home on the second postoperative day. Follow-up showed no recurrence of symptoms. CONCLUSION: Long-term sequelae can occur after type I FGM. The surgical treatment of clitoral inclusion cyst is simple and effective.

5/26. Clitoral cyst as a cause of ambiguous genitalia.

    BACKGROUND: Ambiguous genitalia need appropriate evaluation, because a serious disease can manifest as this condition. CASE: A 28-year-old woman was referred with a "penis" present from age 12 years that enlarged throughout puberty but recently was unchanged. Differential diagnosis included true hermaphroditism; adrenal hyperplasia; clitoral, ovarian, and adrenal neoplasms; stromal hyperthecosis; polycystic ovarian syndrome; and exogenous androgen exposure. Free testosterone was not elevated. dehydroepiandrosterone sulfate, 17-hydroxyprogesterone, total testosterone, androstenedione, deoxycorticosterone, 11-deoxycortisol, chromosomal analysis (46,XX), an intravenous pyelogram, and pelvic ultrasound were normal. The patient elected surgical removal. The mass was a chronic inflamed epidermoid cyst. CONCLUSION: If this patient had been appropriately evaluated earlier, she would have been spared anxiety and embarrassment over a simple epidermoid cyst.

6/26. Late onset vulvar steatocystoma multiplex.

    steatocystoma multiplex is a rare, autosomal dominant condition characterized by multiple cysts, usually arising on the trunk and proximal extremities at puberty. We present two unrelated women with sporadic steatocystoma multiplex strictly confined to the vulva and arising at an older age. These cases represent an unusual localized form of the disorder.

7/26. Bladder perforation presenting as vulvar edema after the tension-free vaginal tape procedure. A case report.

    BACKGROUND: Bladder perforation during the tension-free vaginal tape (TVT) procedure is a common complication. CASE: Vulvar edema was noted 4 hours after application of TVT, and cystoscopy revealed bladder perforation. The patient had a history of a previous vaginal hysterectomy with anterior colporrhaphy. CONCLUSION: To our knowledge, this is the first case report of vulvar edema following TVT. Bladder filling to 250 mL, 70 degrees cystoscopy, good skills in cystoscopy and familiarity with retropubic anatomy are key issues for the TVT procedure. A cystoscopic examination before the TVT procedure should be considered for patients with previous pelvic surgery.

8/26. endometriosis in the Bartholin gland.

    A case of endometriosis infiltrating the Bartholin gland is presented. The initial diagnosis was a Bartholin gland cyst. The clinical diagnosis was made during the operation when chocolate-colored fluid poured into operation field. laparoscopy was done during the same operation and minimal endometriosis was found. The presence of endometriosis may be considered as a diagnosis in cases with cystic mass in the Bartholin gland and laparoscopy might be performed to rule out the presence of intraperitoneal endometriosis if a woman is infertile.

9/26. Fibrocystic disease of vulvar ectopic breast tissue. Case report and review of the literature.

    INTRODUCTION: Mammary glands located in the vulvar region have been named as ectopic breast tissue or anogenital mammary glands by different authors. literature on pathologies of ectopic breast tissue located in the vulvar region is rare. Most of the reports are about the malignancies arising from this ectopic tissue. CASE REPORT: We report a case of fibrocystic disease of the mammary glands in the vulva in a 25-year-old pregnant woman. Her disease was exaggerated during pregnancy. CONCLUSION: Ectopic breast tissue in the vulva is a rare entity and fibrocystic disease of this tissue has rarely been reported in the English literature.

10/26. Multiple pigmented follicular cysts of the vulva successfully treated with CO2 laser: case report and literature review.

    BACKGROUND: A pigmented follicular cyst is an uncommon epithelial cyst, typically occurring as a single asymptomatic pigmented papule or nodule on the head and neck of adult men. To the authors' knowledge, no previous cases of vulvar pigmented follicular cyst have been reported. Although benign, pigmented follicular cyst can affect quality of life when multiple and symptomatic. OBJECTIVE: The objective was to describe the clinicopathologic features of vulvar pigmented follicular cyst and to evaluate the efficacy of CO(2) laser therapy in this region. methods: A 62-year-old woman with multiple pigmented follicular cysts affecting the labia majora bilaterally is described. Owing to frequent inflammation and tenderness, continuous-wave CO(2) laser was used to treat the skin lesions under local anesthesia. RESULTS: All cystic nodules were successfully ablated with CO(2) laser. The patient tolerated the procedure well and had a satisfactory cosmetic outcome. CONCLUSION: CO(2) laser is a tolerable and effective therapeutic modality for multiple and symptomatic pigmented follicular cysts of the vulva.
