1/4. hirsutism secondary to topical testosterone: report of two cases and review of the literature.

    Topical testosterone has been used as a treatment for different vulvar conditions. It is usually well tolerated, although hirsutism and other signs of virilization have been described related to hormonal tests alteration. We report two female patients with vulvar diseases that developed hirsutism after treatment with topical testosterone. Because of its potential virilization, females under this treatment should be closely monitored.

2/4. Further observations on the effect of topical progesterone on vulvar disease.

    follow-up studies have confirmed the beneficial effect of topical progesterone in the treatment of vulvar dystrophy, whether of the atrophic, hypertrophic or mixed variety, in both postmenopausal and premenarchal women. If pruritus is a major symptom, topical corticosteroids should be alternated with the progesterone preparation in order to obtain maximum results. Topical progesterone was also found to be useful in the treatment of labial agglutination in children.

3/4. Pure cutaneous histiocytosis X of the vulva.

    histiocytosis X of the vulva is extremely rare. In most reported cases, vulvar disease is associated with multiple organ involvement and systemic disease manifestations aiding in the diagnosis. Reported is a case of histiocytosis X presenting solely with vulvar lesions for eight years' duration. The diagnosis, pathology, and subsequent management are discussed. The literature of histiocytosis X of the vulva is reviewed, and its presentations and treatments summarized. Vulvar lesions unresponsive to therapy warrant biopsy and other causes of histiocytic reactions must be excluded to establish the diagnosis.

4/4. Nonfluorescent erythrasma of the vulva.

    BACKGROUND: erythrasma is an uncommon vulvar infection, best diagnosed by its fluorescence under the wood lamp. This report shows that despite a negative wood lamp examination, the diagnosis can be made histologically. CASE: A 42-year-old woman was referred to our clinic with a persistent candidal infection. Evaluation included a wood lamp examination, wet mount, and potassium hydroxide test of the affected skin, all of which were negative. A biopsy of the area demonstrated rods and filamentous organisms in the keratotic layer consistent with a corynebacterium minutissimum infection. The patient was diagnosed as having erythrasma, and she responded to oral erythromycin. CONCLUSION: Persistent vulvar diseases may be caused by erythrasma despite a negative wood lamp examination. The diagnosis can be made by biopsy of the lesion.
