1/32. Guess what! Eruptive vellus hair cysts.

    An otherwise healthy 24-year-old woman presented with persistent asymptomatic skin-coloured papular lesions on her trunk which had appeared over 18 months, gradually increasing in number, size and the extent of the skin surface area involved. physical examination revealed multiple flesh - to whitish - coloured papules, 2 to 5 mm in diameter, located on her neck, anterior chest wall, axillae and upper abdomen (Fig. 1). The lesions were smooth, painless on palpation and not grouped. Laboratory investigations only showed normal results. Histopathological examination of a papular lesion from the upper abdomen revealed a cystic structure in the mid-dermis lined by four to five layers of squamous epithelium with a discrete granular layer. Laminated keratinous material and multiple transversely or obliquely cut vellus hairs were present within the cyst, while sebaceous elements were lacking in the vicinity or within the cyst wall (Fig. 2a, b). Polaroscopic examination demonstrated double refractile vellus hairs in the cyst. ( info)

2/32. Eruptive vellus hair cyst and epidermoid cyst in a patient with pachyonychia congenita.

    We report the first case of pachyonychia congenita (PC) associated with both eruptive vellus hair cyst (EVHC) and epidermoid cyst. The patient is a 12-year-old Japanese girl who presented with two natal teeth at birth. She had thickening and discoloration of the fingernails and toenails, plantar hyperkeratosis, palmar-plantar hyperhidrosis and multiple cutaneous cysts. Histologic examination revealed EVHC and epidermoid cyst. ( info)

4/32. follicular cyst in a patient with myotonic dystrophy: a case of cyst with differentiation toward follicular infundibulum, isthmus, inner root sheath, and hair.

    We report a case of follicular cyst, which developed in a patient with myotonic dystrophy (MyD). Histopathologically, the cyst showed infundibular and trichilemmal keratinization, inner root sheath differentiation, aggregation of basaloid cells, and pilomatricoma-like changes in the pericystic connective tissue. These findings have been reported in follicular cysts with Gardner's syndrome (GS). Interestingly, pilomatricoma is known as one of the skin diseases associated with MyD, though there have been no reported cases of cyst formation with differentiation toward portions of hair follicle in a MyD patient. In our case, we hypothesized that the cyst might be derived from embryonic follicular germinative cells or follicular stem cells under the genetic influence of the MyD gene, as observed in follicular cysts in patients with GS. 5/32. Multiple follicular cysts, infundibular type with vellus hairs and solar elastosis of the ears: a new dermatoheliosis?

    BACKGROUND: Vellus hair cyst is an uncommon developmental abnormality of the vellus follicle histologically defined as a stratified squamous epithelial-lined cyst containing one or more vellus hairs. methods: Herein, we report three patients with a heretofore described clinicopathologic condition consisting of multiple contiguous cysts of the ear helices showing solar elastosis and multiple cysts containing vellus hairs, microscopically. CONCLUSION: Given the anatomic location, history of excessive ultraviolet exposure and pathologic alterations, we surmise that this is a condition related to Favre-Racouchot syndrome, which is predisposed by excessive ultraviolet light exposure. 6/32. Perforating follicular hybrid cyst of the tarsus.

    Follicular hybrid cysts are cutaneous tumors that arise from different portions of the hair follicle. We describe a follicular hybrid cyst of the tarsus that contained features of pilomatricoma and steatocystoma and perforated the palpebral conjunctiva. The unusual location and histopathologic features are discussed. ( info)

7/32. Inflammatory follicular cyst associated with an endodontically treated primary molar: a case report.

    This is a case study of an inflammatory follicular cyst associated with an endodontically treated deciduous molar with radiographic and microscopic diagnosis. This cystic lesion represents a typical follicular cyst that arose in association with periapical inflammation from a non-vital deciduous molar and involved the follicle of the unerupted successor premolar. This paper describes the case and discusses diagnosis, histogenesis, and controversies regarding terminology. ( info)

8/32. Multiple pigmented follicular cysts of the vulva successfully treated with CO2 laser: case report and literature review.

    BACKGROUND: A pigmented follicular cyst is an uncommon epithelial cyst, typically occurring as a single asymptomatic pigmented papule or nodule on the head and neck of adult men. To the authors' knowledge, no previous cases of vulvar pigmented follicular cyst have been reported. Although benign, pigmented follicular cyst can affect quality of life when multiple and symptomatic. OBJECTIVE: The objective was to describe the clinicopathologic features of vulvar pigmented follicular cyst and to evaluate the efficacy of CO(2) laser therapy in this region. methods: A 62-year-old woman with multiple pigmented follicular cysts affecting the labia majora bilaterally is described. Owing to frequent inflammation and tenderness, continuous-wave CO(2) laser was used to treat the skin lesions under local anesthesia. RESULTS: All cystic nodules were successfully ablated with CO(2) laser. The patient tolerated the procedure well and had a satisfactory cosmetic outcome. CONCLUSION: CO(2) laser is a tolerable and effective therapeutic modality for multiple and symptomatic pigmented follicular cysts of the vulva. ( info)

9/32. Follicular hybrid cyst (trichilemmal cyst and pilomatricoma) arising within a nevus sebaceus.

    We report a case of follicular hybrid cyst arising in a nevus sebaceus. Histopathologic findings showed one part with the features of a trichilemmal cyst and the other part with those of a pilomatricoma. To our knowledge, this is the first case of follicular hybrid cyst associated with nevus sebaceus. We consider that the cyst in our case is derived from the hair matrix and from the follicular isthmus. ( info)

10/32. Orthognatic therapy for the resolution of a presumed mandibular follicular cyst. Clinical case.

    The case of a presumed mandibular follicular cyst associated with the retention of 4.5 in a 12 year-old patient is described. Objective examination revealed the absence of 4.5 in the dental arch. Orthopantomography revealed a monofocal area of radiotransparency with a sharp outline by 4.5 tooth. The lesion was treated only by fixed multiband orthodontic therapy to reopen the space in the dental arch. This treatment permitted the total remission of the cystic lesion without any surgical action, as confirmed by the following radiographical exams. ( info)
