are repeat lumpectomies ever done for a local recurrence?


my mom is 80. last year she had a lumpectomy with chemo followed by femera. no radiation. the tumor was the most aggressive type.
her recent mammo showed a local recurrence of the tumor in the same area. it is not a new primary tumor.
her original surgeon says he would do another lumpectomy and radiation. another surgeon recommends a mastectomy. my question is whether second lumpectomies are done and what the outcomes are compared to mastectomy. i hate to see her go through such a radical surgery at her age..any feedback would be much appreciated.

i'm not sure about second lumpectomies but i work for a surgeon and in cases of recurrence such as this he recommends mastectomies. it is a big surgery but if it already came back one time, it could very well come back again and again in different areas which would mean even more surgeries. i'm sorry i don't know what the outcome stats are. could you speak to your family doctor to get a third opinion on what would be the best way to go??  (+ info)

is thyroidectomy the ONLY way to determine if a papillary lesion / follicular neoplasm is benign or malignant?


My wife recently underwent FNAB for the thyroid gland and the findings were:
1. PAPILLARY LESION
2. FOLLICULAR NEOPLASM.
Suggest thyroidectomy for a definitive diagnosis.
Microscopic Description:
Smears disclose a fairly cellular aspirate composed of cohesive clusters of follicular cells, in attempt to form acini and short papillary fronds. The cells show vesicular nuclei, with focal areas of pleomorphism. The background is hemorrhagic containing thin colloid materials and few mixed leukocytes.

I really would like to know if the it is benign or malignant but is there any other way besides invasive surgery? Thanks a lot in advance for all the answers and help.

If FNAB demonstrate follicular neoplasm , we must perform thyroid lobectomy for determining if it's malignant or not , and regarding to this , we will design the further definite and main operation .

There is no way other than this yet .

In some situations , we can perform total thyroidectomy as a plan to determine the permanent pathology at first ( there is several indications : old patients , mass more than 4 cm , ... ) .

But about Papillary neoplasm it's somewhat different : we can plan for a definite operation , also with a FNAB .  (+ info)

What is the length of time for the sypmtoms of a recurrence of herpes 2?


I have some tenderness but no visible discoloring or rash around a patch of skin on my right upper thigh. This has been going on for about 5 days. After doing some internet research, I think I might be one of the millions of people who have some form of herpes but do not know it due to very mild, almost symptomless recurrences. I have linked my skin tenderness to a possible herpes infection but I can't find any hard information on a typical recurrence's duration.

There is no set time period for HSV-2 recurrence. They can be triggered by physical or mental stress, other illnesses (cold, flu, etc.) but other than that, the recurrences are quite random. There are no studies that show clearly documented triggers. Herpes typically has a vesicular appearance which means that the pump has a clear fluid filled center. These lesions are typically preceeded by tingling, pain, or tenderness to the inguinal area (most often). If you want to know for sure, I would suggest going to your primary care doc and having him run a blood test for HSV-2. That way you know for sure what you're dealing with.

If you're relatively asymptomatic, then you may not require treatment, unless you're sexually active. The funny thing about HSV-2 that you don't hear on TV or in sex ed class is that contact with the lesions or their fluid can cause transmission. The lesions are not always found on the penis and can be in the inguinal or inner thigh region where condoms do not reach. So the best thing to do if you have a confirmed infections is to be on suppressive medication. This can be done with Valtrex, once a day (i'm sure you've seen the commercial) or with Acyclovir twice a day, which is quite a bit cheaper if you don't have a good prescription plan on your insurance.

Does the vaccine help to reduce recurrence for herpes simplexs 1 in eye? Or any other way to reduce Recurr?


I heard there is vaccine for herpes simplex virus but does it help for the patient that have hsv1 in corneal? Is there any ways to reduce the recurrence for the hsv1 in eye?

The vaccine won't help or protect those who have the virus already. Suppression therapy, such as Valtrex daily can reduce the outbreaks and severity.  (+ info)

What are the chances of a breast cancer recurrence? and survival rate?


a close person i know has Invasive ductal carcinoma in situ with minimal/ inconclusive shes her2+ tumor is 1.5 cm. She's getting a second opinion from somwhere before she begins lumptectomy and radiation followed by chemotherapy. I would like to know how long can she live upto and the chances of a recurrence.

There are lots of statistics out there, but I hate to try and go by that because everyone is different. There are so many factors that go into each case. I was diagnosed with invasive ductal carcinoma last year. On the mammogram it appeared to be 1.5 cm, but by the time I had the tumor removed it was 2.5 cm. I am also her2+ and Er/Pr+. I chose to have a mastectomy, and I had node involvement so I also had chemotherapy and radiation. Because I was Her2/neu+ I had a year of Herceptin treatments. My last treatment is in two weeks. Herceptin was approved not that long ago to use in treatment for eary stage Her/2neu+ breast cancer. I would assume your friend will be having that part of her treatments. There are many treatments available to us and there are many many survivors. I'm glad to hear she's getting a second opinion, I think that's so important. I wish your friend the best.  (+ info)

How local does honey need to be to potentially help with allergies?


I recently had my first allergy test and learned that I'm allergic to dust mites, cats (sad!! - I have two!) and ragweed. In anticipation of the ragweed season being six months away or so, I wanted to start trying local honey now to see if it helps with my ragweed allergy. I know it has to be unpasturized honey, but how local does it need to be? I found a farm within about fifteen miles - is that close enough?

Fifteen miles is close enough. Even further would be okay. Honey is a natural product and it is not processed in any way other than to remove the comb. You may not even have to travel to a farm to get local honey. I purchase local honey at the supermarket where I shop. It's quite common for bee keepers to distribute honey throughout their area.  (+ info)

How do you eat green and support local farms from a practical, day to day standpoint?


I recently watched the documentary "Food Inc" and was really disturbed by what I saw. I want to eat seasonally and buy food from local farms here in New Jersey, but it all seems so complicated. Either to have to buy a big amount (certainly of meats) or wait for the Farmer's Market once or twice a week. Is there an easier way somewhere online to buy local, organic cruelty free food? I live in Hoboken.

It's great that you have seen Food Inc and it has opened your eyes.

Eating locally straight from the farm is a lot more inconvenient than just going to the grocery 24/7. But it is this no thinking convenience that got us into trouble in the first place. When food got to easy to access we quit thinking about it or considering it's importance. And this allowed the food corporations to develop factory farming/industrial food and to keep the where and how of our food system a complete mystery to most eaters.

So from the convenient industrial system you get mystery food like substances but it is cheap (well its only cheap because out tax money pays for the billions in farm subsidies to make industrial processed food seem cheap), we don't have to know how to actually cook to prepare these items for eating (ready to eat/heat and eat) and it is available all over the place all the time.

But now you know that crap is bad to eat and you want to eat better. Welcome to my world. I am a small farmer who sells direct to my customers via farmers markets, farm store and a CSA. I also eat 90% of my diet locally and I stay away from anything with HFCS, GMO's or Hydrogenated oils which is pretty much all processed foods.

One of your problems is you picked the worst time of year to go local. We are about 6 to 8 weeks from the start of the harvest season. There are not many farmers markets open until May or June. there is not much choice in the food this time of year-meat, leafy greens some roots like parsnips and potatoes. Maybe some winter squash still

First check out http://www.localharvest.org for a list of all the farmers markets within 30 miles of you. You will have to get used to the fact you will have to pre plan your meals and buy most of what you need from local sources once or twice a week.

Right now I have a monthly farmers market so I buy the meat I need for the coming month. This is not hard to do but will take some practice if you are used to going out to the store any time you felt you needed food. if you feel you need to shop daily be prepared to drive a lot every day sourcing food from different farms. I think you find it much more efficient to find a couple of farmers markets to go to every week and start thinking ahead at least a few days.

You can also grow some of you own if you have any kind of balcony or better a yard. You do not need land. All you need are some largish pots to plant vegetable seeds into and than months of TLC. You won't be able to grow more than maybe 1% to 2% of what you eat but it will give you a better understanding of what we do in order to eat.

And remember you are buying direct from farmers. We are not an entity like Wal-Mart, we are real flesh and blood people who work long, often brutally hard hours to produce sublime food. Usually pretty much by hand, on small acreage for far less money than most suspect. We do not have a PR staff, marketing staff, etc. And that is why we are not open 24/7. if we were it would kill us all and than you have no option other than industrial factory farmed food. So realize this is something you will have to get used to in order to participate.

Joan, I respectfully have to disagree that there is no green food. I have been growing such for over 15 years. There are profound difference as to how food is grown between industrial farms and small farms. True not all are organic and you do have to do a lot of your own investigative foot work. But there are differences.

I also take issue that you can wash away all the bad stuff. Wrong. You cannot wash away GMO's they are a part of the DNA. And you cannot wash away a lot of pesticides as they become a part of the food (i.e. they get absorbed by the plant). Not to mention when you wash the chems off they just go into the water system so you end up contaminating the water supply unknowingly.

Joan, you need to see Food Inc and learn a few things about the Industrial food system where 98% of the US Population gets its food. You will se the system is broken and that the corporations who feed us care far more about profits and shareholders than about our health.  (+ info)
