What is pellagra and how is it treated?


Background: Pellagra defines the systemic disease resulting from niacin deficiency. Don Gasper Casal, a Spanish court physician, first described pellagra. In 1735, Casal observed cases of pellagra among the poor peasants of the Asturias. In an Italian vernacular, pellagra means "rough skin" and refers to the thickened rough skin of the persons with pellagra. Pellagra remained endemic among the maize-eating poor peasants of southern Europe for nearly 2 centuries before being recognized in the United States.

Medical Care: PO nicotinamide or niacin usually is effective in reversing the clinical manifestations of pellagra. As patients often are malnourished and suffer from other vitamin deficiencies, provision of a high protein diet and B-complex vitamins is needed for complete restoration of health.

Diet: In order to prevent and/or treat pellagra, provide a diet high in protein and adequate in calories. The addition of meats, milk, peanuts, green leafy vegetables, whole or enriched grains, and brewers' dry yeast can enhance the niacin intake. In patients with oral dysphagia secondary to glossitis, a liquid or a semisolid diet may be required. Long-term inclusion of milk, meat, and eggs in the diet ensures dietary adequacy of proteins essential for recovery.

Activity: Bed rest is mandatory in treatment of severe cases of pellagra.  (+ info)

Pellagra in babys mouth?


I have a 18 month old baby and the corners the mouth are cracked, which appears to be pellagra. What can i do to treat it, and what type of food should i give. I went to the doctor and they prescribed anti-fungal. What else can i do?

Keep mouth area dry. Change face towels often. Child should not go into a pool or anywhere else fungus festers. Get a clear diagnosis. Get a topical cream/ointment as well.  (+ info)

Pellagra is a vitamin deficiency disease caused by dietary lack of niacin (vitamin B3) and protein, especially proteins containing the essential amino acid tryptophan. Because tryptophan can be converted into niacin, foods with tryptophan but without niacin, such as milk, prevent pellagra. However, if dietary tryptophan is diverted into protein production, niacin deficiency may still result.

Some sources also claim a relationship between lysine and pellagra, but this position does not have nearly as much support.


Symptoms

The symptoms of pellagra include:

* high sensitivity to sunlight
* aggression
* dermatitis
* red skin lesions
* insomnia
* weakness
* mental confusion
* diarrhea
* dementia (eventually)

The main results of pellagra can easily be remembered as "the four D's": diarrhea, dermatitis, dementia, and death.

Epidemiology

Pellagra can be common in persons who obtain most of their food energy from corn, since untreated corn is a poor source of niacin (vitamin B3). Corn is also a poor source of tryptophan. This disease can be common among people who live in rural South America where corn is a staple. The symptoms usually appear during spring, increase in the summer due to greater sun exposure, and return the following spring. It is also one of several diseases of malnutrition common in Africa, and was endemic in northern Italy, Spain, and southeastern Europe. Alkali treatment of the corn corrects the niacin deficiency, and was a common practice in native American cultures that grew corn. The amino acid deficiency must be balanced by consumption of other sources of protein. It was also common amongst prisoners of Soviet labor camps, the infamous Gulag.

EDIT- Note to second poster...sometimes this and other similar "third world" diseases are seen in conjuction with anorexia or alcoholism...among maybe some other scenarios.. I agree that niacin supplements should be first on the list to correct this problem...

Anything with niacin (B3) or tryptophan in it (tryptophan converts to niacin in the body (ie milk or turkey).
