11/30. Delivery of a healthy twin after intrauterine demise and expulsion of a first twin.

    We report on a diamniotic-dichorionic twin pregnancy after in vitro fertilization. The first twin was diagnosed with down syndrome and spontaneously aborted at 24 weeks of gestation after intrauterine death at week 18. The second healthy twin was delivered by cesarean section 11 weeks later. We discuss management aspects and review the literature.

12/30. Persistent gestational trophoblastic disease after an androgenetic/biparental fetal chimera: a case report and review.

    We present a case of a dichorionic/diamniotic twin pregnancy in which one twin presented with ultrasound findings suggestive of molar changes in the placenta. The placenta of twin A seemed to be grossly enlarged and cystic, and twin A was small for gestation. After an inevitable abortion, a detailed histological and genetic evaluation was performed on the fetus and placenta from twin A, including traditional cytogenetic techniques, microsatellite marker analysis, fluorescent in situ hybridization, and p57 immunostaining. It was determined that twin A was a chimera with a biparental XX cell line and an androgenetic XY cell line. The 2 cell lines were present in both the placenta and the fetus. The patient later developed and was treated for persistent gestational trophoblastic disease, which has been shown to have an increased risk after an androgenetic conception. Cases of mosaicism or chimerism involving an androgenetic cell line may be difficult to diagnose histologically but are critical to identify because of the increased risk for persistent gestational trophoblastic disease. Therefore, we emphasize the importance of using multiple molecular, cytogenetic, and immunohistochemical techniques when diagnosing cases involving such unusual placental abnormalities. To our knowledge, this is the first reported case of persistent gestational disease after a fetal chimera.

13/30. A pelvic pseudoaneurysm (a rare complication of oocyte retrieval for IVF) treated by arterial embolization.

    This is a case report of the diagnosis and treatment of a pelvic pseudoaneurysm in a patient. Her history leads to the conclusion that this developed following egg retrieval during in vitro fertilization six years earlier.

14/30. Pregnancy following regression of uterine submucosal leiomyoma with GnRH therapy; a case report.

    A patient with a history of two spontaneous abortions, at the 16th and 20th week, respectively, and one intra-uterine fetal death at the 26th week of gestation was investigated. The sole abnormal condition that could be evidenced was a submucosal leiomyoma of 56 mm in diameter in the uterine fundus. Administration of gonadotropin/releasing hormone analog (GnRHa) for 10 months resulted in complete disappearance of myoma. Direct intraperitoneal insemination following induction of ovulation resulted in pregnancy. In a patient harboring uterine leiomyomas, adversely affecting conception and pregnancy outcome, GnRHa treatment may be an initial approach allowing to avoid pelvic surgery. As the beneficial effect of GnRHa might be temporary, assisted reproduction procedures might anticipate conception.

15/30. Triple autosomal trisomy in a pregnancy at risk for Bloom's syndrome.

    cytogenetic analysis of the products of conception in a pregnancy at risk for Bloom's syndrome (BS) documented the karyotype 49,XX, 2, 8, 11. Autosomal triple trisomy has previously been reported in abortuses but is exceedingly rare. Other interesting but previously unreported observations made during the present study were the following: BS in a Brazilian individual, the first instance of BS diagnosed in south america; transmission of the BS mutation in jews that are non-Ashkenazi; a medulloblastoma in the propositus, the first malignant brain tumor reported in BS; and, as in all previously examined pregnancies at risk for BS, non-homozygosity for the BS mutation.

16/30. Very high CA 125 levels during early first trimester in three cases of spontaneous abortion with chromosomal abnormalities.

    Three women with very elevated, early-first-trimester CA 125 levels spontaneously aborted but not until later in the first trimester or early in the second trimester. All three products of conception showed chromosomal abnormalities. Further investigation is warranted to see if high CA 125 levels might be predictive of abnormal karyotypes.

17/30. in vitro fertilization with spermatozoa from alloplastic spermatocele.

    This case report describes successful IVF of oocytes of a 22-year-old female with epididymal spermatozoa aspirated from alloplastic spermatocele of the husband presenting with bilateral congenital absence of the vas deferens. Motile spermatozoa were aspirated from the reservoir 3 months after implantation. Abortion occurred 22 days after embryo replacement.

18/30. ovarian hyperstimulation syndrome (OHS): a case of severe OHS.

    A 26 year old patient, who underwent an ovulation stimulation therapy (clomiphene /UH-FSH), monitored only by ultrasound presented a severe ovarian hyperstimulation syndrome (OHS). beta-HCG values confirmed conception, that outcame in an abortion. During an ovulation stimulation therapy, patient's management with the ultrasound examination, is often inadequate and it's moreover necessary to determine the 17 beta E2 levels.

19/30. The outcome of pregnancy after chemotherapy for gestational trophoblastic disease.

    Thirteen pregnancies in 11 women previously treated with cytotoxic chemotherapy for GTD were studied. There were 11 live full-term babies (84.6%), 2 blighted ova (15.4%), 1 placenta accreta (7.7%) and 1 fetal anomaly (7.7%). Unlike other series, 69% of the pregnancies occurred within 1 year after the termination of chemotherapy. The rate of successful pregnancy was not different from that reported by others. Prenatal elimination of abnormal embryos after conception rather than wastage of damaged oocyte before conception might explain this observed low incidence of abnormal birth in GTD patients treated with cytotoxic chemotherapy.

20/30. Immunochemical and biological characteristics of a human autoantibody to human chorionic gonadotropin and luteinizing hormone.

    Anti-hCG/LH autoantibodies were found in the serum of an infertile woman a few days after an abortion which occurred after 46 days of amenorrhea. The antibody titer increased for approximately 4 more weeks, and then declined to low levels during a 14-month anovulatory period, after which regular menses resumed. immunoglobulins isolated from a pool of serum obtained during the postabortion period neutralized the activity of both hCG and LH in an in vivo bioassay, and the binding affinity of the antibodies toward both hormones was high. When menses were resumed, there was a considerable reduction of the affinity toward LH. The variations in antibody titers and/or affinities can explain the sequence of fertilization, abortion, anovulatory period, and normalization of menses.
