11/51. Warthin's tumor of parotid gland: treatment of a retroneural lesion by enucleation.

The papilliferous cystadenomas lymphomatosum or Warthin's tumor is a relatively rare and generally benign neoformation whose incidence is second only to the pleomorfic adenoma. 84% of all Warthin's tumor strikes the parotid gland. We report the two cases that come to our observation paying particular attention to the second one in which occurred a synchronous bilateral manifestation, a rarity in the clinical circle. We want to highlight that enucleation should be the preferred treatment in most cases due to extremely small dimension of Warthin's tumor. Moreover it is possible to perform this kind of surgical treatment even in the case of a retroneural lesion without causing any damage to the facial nerve and its main branches. As regards complications and sequelae we noticed neither a permanent nor a temporary injury of the facial mimicry and sensibility.

12/51. Malignant Warthin's tumour: a case study.

Malignant transformation of a benign Warthin's tumour (adenolymphoma) is extremely rare. The light microscopic and ultrastructural features of an adenocarcinoma arising in a Warthin's tumour in the parotid gland are described. light microscopy demonstrated a transition zone from the benign to the malignant component, and the malignant cells revealed oncocytic features by both light- and electronmicroscopy.

13/51. A trial for histopathological subclassification of papillary cystadenoma lymphomatosum by 99Tcm-pertechnetate in a patient with multiple bilateral lesions of the parotid glands.

OBJECTIVES: The purpose of this report was to evaluate the possibility of subclassification of papillary cystadenoma lymphomatosum (PCL) with 99Tc(m)-pertechnetate. methods: A patient with multiple bilateral PCLs in the parotid glands was examined by using 99Tc(m)-pertechnetate. RESULTS: All PCLs of the present case, which were diagnosed as the subtype-II histopathologically, showed similar radioactive indexes in scintigraphy (the mean radioactive index = 3.62), although tumours were different in size. The mean radioactive index corresponded well with that from four cases of subtype-II of our previous report (the mean radioactive index = 3.84). CONCLUSIONS: The results of the present report suggest a possibility of histopathological subclassification of PCLs into subtypes by 99Tc(m)-pertechnetate scintigraphy.

14/51. Malignancy of a cystadenolymphoma of the parotid gland.

The authors describe malignant degeneration of the epithelial component of a cystadenolymphoma (CAL) of the parotid gland in a 79-year-old man. This development of carcinoma in CAL is a very rare occurrence, with only eight cases described thus far in the literature.

15/51. Papillary cystadenoma lymphomatosum: case report and review of literature.

Papillary cystadenoma Lymphomatosum or Warthin's tumour is a unique neoplasm arising almost exclusively in the parotid salivary gland and is composed of oncocytic epithelium with a prominent lymphoid infiltrate. The tumor is usually asymptomatic and may be associated with varying degrees of pain not necessarily an indication of malignant transformation in the tumour. The tumour is managed easily by surgical excision. This article presents a typical case arising in the right parotid gland of a 73-year-old male with review of literature of this rare tumour.

16/51. Unilateral multiple tumours of the parotid gland.

The occurrence of two morphologically distinct tumors in a single parotid gland is rare. Pathologic examination of a unilateral parotid mass removed from a 63-year-old male revealed the presence of a benign mixed tumor and an adenolymphoma. No other examples of this relationship were encountered in the literature.

17/51. Bilateral papillary cystadenoma lymphomatosum of the parotid gland without accumulation of technetium 99m pertechnetate: report of a case and review of the literature.

An unusual case of bilateral papillary cystadenoma lymphomatosum of the parotid with multiple lesions showing no active uptake of 99mTc pertechnetate is presented. Similar cases showing no accumulation of radionuclide are reviewed.

18/51. Unilateral multicentric Warthin's tumors.

Papillary cystadenoma lymphomatosum, or Warthin's tumor, is known to have a high rate of multicentricity. An incidence of 5-14% is suggested by the literature. We present a female patient with three discrete masses noted in the superficial lobe of the parotid gland at the time of surgical resection. The epidemiology, histology and pathophysiology of Warthin's tumor is discussed and the need for adequate surgical resection is emphasized.

19/51. Follicular non Hodgkin's lymphoma in adenolymphoma: report of a case.

A case of centroblastic centrocytic follicular lymphoma arising within a bilateral adenolymphoma of the parotid glands with involvement of the adjacent lymph nodes is reported. The patient was treated with combination chemotherapy and three years after the first cycle of therapy there was no evidence of disease. The occurrence of primary malignant lymphomas in the lymphoid stroma of an adenolymphoma has seldom been discussed in literature. All cases reported of follicular centre cell type were characterized by an indolent clinical course.
