1/6. Custom skiing and trekking adaptations for a trans-tibial and trans-radial quadrilateral amputee.

    A keen skier who is a trans-tibial and trans-radial quadrilateral amputee sought an improved adaptation for skiing from the rehabilitation engineering Service in Edinburgh. The unpredictable nature of the bending moments and loads that can be imposed on the prostheses during skiing raised concern about the suitability of standard prosthetic components for this purpose. The authors report a ski boot modification that incorporates mechanical protection for the standard prosthetic components and a description of the custom-adapted alpine trekking sticks used also as ski poles. Reference is made to the role of risk assessment, the design and manufacture in providing this type of custom-made rehabilitation device.

2/6. Fitting a bilateral transhumeral amputee with utensil prostheses and their functional assessment 10 years later: a case report.

    Traumatic transhumeral amputations resulting from electric burn injury are uncommon and present a significant rehabilitation challenge. Compensating for loss of fine, coordinated function of the upper extremities with prostheses is difficult medically, technologically, psychologically, and socially. We followed up a patient with traumatic bilateral transhumeral amputation who was fitted with specially designed bilateral low-temperature utensil prostheses for 10 years. A bilateral utensil prosthesis consists of 2 thermoplastic sockets, an elastic harness, 2 utensil holders, and several different utensils. The characteristics of utensil prostheses are low cost, quick fabrication, and responsiveness to a patient's needs. Ten years after the patient's first prosthesis fitting, he still used these specially designed prostheses. By using these devices and his feet, the patient has regained independence in most activities of daily living and gained a new working skill.

3/6. Prosthetic rehabilitation in bilateral high above elbow amputation.

    rehabilitation in patients with bilateral high above elbow amputation presents a considerable prosthetic problem. A patient with high upper arm amputations after a high-voltage injury is presented. He was successfully fitted with multifunctional myoelectric hand prostheses. The problems in rehabilitation of adult bilateral arm amputees are discussed and the value of fitting these patients with electrically powered prostheses is assessed. The balance between technical and clinical aspects is discussed in relation to patient acceptance. In our case good acceptance and functional benefit was noted. The fact is stressed that the bilateral upper extremity amputee can regain considerable physical function with the fitting of suitable prostheses, even if the limb remnants are short and provide little or no function. An extensive team approach at specialized centres will favour the results.

4/6. pain and its absence in an unfortunate family of amputees.

    A family is described in which 5 male members sustained major traumatic injuries of their limbs. Two of these men had amputations of two of their limbs. The one surviving amputee is left handed. The development of phantom sensations, phantom pain and stump pain was unpredictable, despite their being first-degree relatives, and was independent of handedness.

    This report describes the application of occupational therapy to a patient with traumatic bilateral shoulder disarticulation. Treatment goals and methods for acute care, preprosthetic training, and prosthetic training are described and illustrated.

6/6. Above-elbow amputation: a case study in restoring function.

    Functional goals and treatment are the basis for occupational therapy. Following upper extremity amputation patients can benefit significantly from a program that concentrates on setting and achieving functional goals. Few hand therapists have the opportunity to treat a large number of amputees. This case study of treatment of above-elbow amputation discusses the use of an activities of daily living form as an aid to developing and meeting short-term goals in treatment planning.
