Prostaglandin levels in endometrial jet wash specimens in patients with dysmenorrhea before and after indomethacin therapy.A patient with functional primary dysmenorrhea of over two years duration was subjected to the endometrial jet wash technique during the period of active menstrual flow. Prostaglandin F analysis of the jet washings revealed significantly elevated levels during menstruation over normal control levels. Following indomethacin therapy, jet wash prostaglandin F levels were dramatically reduced and the patient became asymptomatic. Case of iatrogenic dysmenorrhea in non-communicating rudimentary uterine horn and its laparoscopic resection.Abstract A case of non-communicating rudimentary uterine horn is presented and is a characteristic example of the way that surgical treatment can exacerbate dysmenorrhea by blocking retrograde menstruation. There is always some risk of missing the true diagnosis in cases of uterine abnormalities, even by direct inspection, but intraoperative laparoscopic ultrasonography can be an invaluable tool in defining anatomy. Symptoms associated with menstruation.Symptoms associated with menstruation during the teenage and young adult years may represent a spectrum of possibilities varying from a relatively benign deviation from normal to a serious life-threatening disease. Nevertheless, even for the young woman found to be without serious disease, menstrual problems have special meaning associated with the fear of being different from peers, concern about being less than complete or wholesome, and anxiety regarding future functioning as a normal woman. The physician must be aware of the adolescent's sensitivity in this area. Her apprehension should influence the manner in which the various abnormalities of menstruation are evaluated and managed. The physician must understand and appreciate the potential for profound emotional effects and psychological implications of menstrual conditions. Self-mutilation, anorexia, and dysmenorrhea in obsessive compulsive disorder.This report described 19 female patients (M = 23.5) diagnosed as obsessive compulsive disorder (OCD; DSM-III-R) who exhibited additional symptoms of self-mutilation, dysmenorrhea, and dysorexia. A biphasic pattern related to menstruation during the course of OCD emerged: Phase 1, amenorrheic--characterized by anorexia nervosa, amenorrhea, and aggressive behavior, and Phase 2, postamenorrheic--characterized by self-mutilation following the return of the menstrual cycle, dysorexia, and aggressive behavior. All mutilative acts were reported by the patients to be painless and consisted of slashes. Seventy percent of the patients were sexually abused during childhood. All patients underwent an open trial of clomipramine (M = 200 mg/day) for 6 months, and intensive behavior therapy for 8 weeks. Based on clinical observations and self-reports, there was a decrease in self-harm and OCD symptoms. The emergence of OCD, self-mutilation, dysorexia, and dysmenorrhea in a sequential manner may suggest a specific clinical syndrome or the presence of an OCD subset. 6/8. adenomyosis following endometrial rollerball ablation.Two cases of adenomyosis following endometrial ablation using a rollerball electrode are reported. In both patients the endometrial ablation was performed for severe menorrhagia in the absence of uterine pathology. The endometrium was not prepared. After the operation, the women had persistent light menstruation for 7 to 9 months which then became heavy. They also developed progressive dysmenorrhoea and uterine enlargement. Abdominal hysterectomy was subsequently performed and adenomyosis was confirmed on histological examination. Microwave diathermy treatment for primary dysmenorrhea.This case report documents the use of microwave diathermy in a 31-year-old woman who had had primary dysmenorrhea since menarche began at age 13 years. For 18 years, she had severe monthly pain, frequently resulting in emergency department admissions and 1 to 3 days lost from work. Conventional treatments, including pain-relieving drugs, anti-inflammatory drugs, muscle relaxants, superficial heat, and oral contraceptives, had all been unsuccessful in relieving or abating the intense and debilitating pain. Microwave diathermy (45 W total power) was administered for 20 minutes each month on the day symptoms began (usually the first day of menstruation). Over a 7-month interval, diathermy was followed by almost-immediate and long-lasting relief of symptoms. During the 7 months of treatment, the patient lost no workdays due to severe pain. Cutaneous reaction to naproxen.A 28-year-old woman with a previous atopic history had been complaining of itching and burning erythematous plaques or blister eruptions on her face and neck for the last 2 years. These lesions became red-brown and then disappeared in 1-2 weeks. However, the site of two of them had remained heavily pigmented after resolution. Sometimes, vesicular lesions affected the oral mucosa causing a burning sensation. She had noticed that these eruptions reappeared in the same location and related to menstruation (when she used to take naproxen sodium because of dysmenorrhea). Furthermore, pigmented sites became red-brown, elevated and itchy. These findings suggested a fixed drug eruption (FDE) due to naproxen, a sporadic clinical event previously reported only once. patch tests were performed on the back (normal skin) with a series of NSAIDs, and with naproxen both on the back and on previous FDE sites. The test were negative on the back, and on previous FDE sites the skin got dark.
