1/8. "Pure word deafness": implications for assessment and management in communication disorder--a report of two cases.

    In "pure word deafness" after acquired brain injury, the auditory comprehension of words is much more impaired than other aspects of communication or cognition. Two cases are presented, one early and one late presentation. The key to diagnosis of communication disorders is to remember to assess all six basic aspects of language function and to be vigilant for coexisting diagnoses that can complicate such assessment (especially psychiatric diagnoses). rehabilitation management of impaired communication should emphasize the teaching of specific coping mechanisms to the patient and to all others who are involved.

2/8. growth of totally thrombosed giant aneurysm within the posterior cranial fossa. Diagnostic and therapeutic considerations.

    We report a case in which growth of a totally thrombosed giant aneurysm of the posterior cranial fossa was demonstrated by computed tomography (CT) scans repeated after 4 years. A magnetic resonance (MR) image demonstrated an onion-skin-like, laminated structure within a calcified wall. The laminated structure had developed around an old thrombosis, without any communication to the flowing blood. It showed intensities indicating recent clots, revealing that the giant aneurysm had grown by recurrent intramural hemorrhage rather than by intraluminal accumulation of thrombotic materials. This case illustrates that totally thrombosed giant aneurysms still have the potential of growth.

3/8. Body communication as compensation for speech in a Wernicke's aphasic--a longitudinal study.

    This case study describes a Wernicke's aphasic who had spontaneously developed a communication pattern involving a large portion of body communication to convey factual information. The study is longitudinal and compares the development of body communication and speech (here: the use of nouns, verbs, adjectives, and pronouns) during the 18-month period of rehabilitation. There is a clear development, showing an increased use of nouns, verbs, and adjectives and a decreased use of body communication to convey factual information. This is taken to show that compensatory body communication for factual information can be used by Wernicke's aphasics, something that is easily obscured by the finding often reported that most aphasics tend to show body communication patterns that are in accordance with their speech patterns. The role of pronouns and gestures for turnkeeping at an intermediate stage, when the patient is shifting from gestures to words for factual content, is also discussed.

4/8. Dural arteriovenous malformation of the major venous sinuses: an acquired lesion.

    arteriovenous malformations of the dura are thought to be congenital. However, arteriographic investigations of four patients who, after a head injury, developed dural arteriovenous fistulae with features of congenital malformations suggest that these abnormal communications may also be acquired. thrombosis or thrombophlebitis in the dural sinus or vein may be the primary event in their formation. The pathogenesis is probably "growth" of the dural arteries normally present in the walls of the sinuses during the organization of an intraluminal thrombus. This may result in a direct communication between artery and vein or sinus, establishing an abnormal shunt. Ultimate fibrosis of the sinus wall and intraluminal thrombus may be the factors responsible for the spontaneous disappearance of such malformations.

5/8. Fatal cerebral air embolism during open heart surgery caused by lung parenchyma to left atrial communication.

    We present an unusual sequence of events leading to massive cerebral air embolism and death in a patient who underwent mitral valve re-replacement for failure of the previously inserted bioprosthetic valve. An iatrogenic communication was created inadvertently between the adherent lung parenchyma and the left atrium. It caused a massive influx of air from the lung into the left atrium and systemic circulation causing fatal cerebral embolization. Retroperfusion was not effective.

6/8. Spontaneous thrombosis of a traumatic cavernous sinus fistula.

    Post-traumatic thrombosis of cavernous sinus and orbital veins has been reported in conjunction with carotid-cavernous sinus fistula (CCSF). CCSF are abnormal communications between the carotid artery and the cavernous sinus. Spontaneous thrombosis of a CCSF is a rare event mainly seen in the indirect (dural) type of fistula. We report on a patient with a possible post-traumatic CSSF which underwent partial spontaneous thrombosis and presented with an extreme degree of proptosis.

7/8. Perioperative stroke in patients undergoing head and neck surgery.

    The risk of perioperative strokes has been demonstrated to be very low in general surgical procedures, and somewhat higher in cardiac and carotid artery procedures. We describe 5 patients who underwent major head and neck procedures not requiring carotid ligation and who postoperatively suffered strokes. These occurred between the first and ninth postoperative days. Four of the patients were thought to have had emboli, 3 to the cerebral hemispheres (2 ipsilateral and 1 contralateral to the neck dissections), and another to the lower brain stem. Hypoperfusion was thought to have caused the stroke in the fifth patient. All patients had risk factors for stroke. The cases in our series were difficult to diagnose because of the delayed onset and subtle nature of symptoms, as well as masking of speech and communication due to the operative involved. Thrombogenesis within the internal carotid and vertebral artery systems due to patient positioning and intraoperative cervical manipulation may be an important etiologic factor in this form of stroke.

8/8. Detachment of defective septum primum. A potential source of embolism?

    Cardiac sources of embolic stroke include various abnormalities of the atrial septum. An atypical detachment of the septum primum was detected in two patients by dynamic three-dimensional echocardiography (DTDE). Between the septum primum and septum secundum a cavity with interatrial communication was detected. Within the cavity a turbulent color Doppler signal and an entry of the contrast agent EchovistR were detected. One of the patients suffered from repeated cerebrovascular events. This unusual defect of the atrial septum may be a potential source of embolism. Transesophageal echocardiography is considered the most sensitive method for the examination of the atrial septum. However, clear anatomic details could only be shown by DTDE.
