Renal masses in this age group are often cystic and may be associated with other abnormalities. Percutaneous antegrade pyelography in small infants and neonates.Percutaneous antegrade pyelography is a safe and useful alternative to retrograde pyelography in the investigation of urinary-tract malformations in the neonate or very young infant. It is preferred to arteriography in the infant with a loin mass. Satisfactory delineation of hydronephrosis or cysts is simply and directly accomplished by this method, so that more complex and less definitive investigations can often be avoided. The most common cause of pseudocyst formation is trauma. hydronephrosis is almost always associated with urinary pseudocyst formation. Treatment should be directed at relieving urinary obstruction with excision of the cyst wall when renal parenchyma remains. 4/29. survival in second trimester oligohydramnios secondary to bilateral pelviureteral junction obstruction.The extent and onset of obstruction in hydronephrosis determine the varying degrees of renal impairment. Bilateral hydronephrosis, especially in combination with oligohydramnios, is considered a negative predictor for pregnancy outcome. We describe a case of bilateral pelviureteral junction obstruction causing severe oligohydramnios between 25 and 29 weeks of gestation. The prenatal and postnatal findings and treatment are demonstrated. In the presence of bilateral renal impairment and oligohydramnios, our patient had an unfavorable prognosis. The respiratory and renal function, however, were better than expected. 5/29. nephrocalcinosis and medullary cysts in 3-methylglutaconic aciduria.3-methylglutaconic aciduria is frequently found during urine organic acid analysis and is widely regarded as a marker of a mitochondrial disorder, the clinical features of which are very heterogeneous. We describe two siblings with 3-methylglutaconic aciduria in whom renal ultrasonography showed echogenic medullae consistent with nephrocalcinosis. One patient also developed medullary cysts. In both children renal function was normal and neither had any plasma or urinary evidence of tubulopathy. 6/29. candida krusei renal cyst infection and measurement of amphotericin b levels in cystic fluid in a patient receiving AmBisome.candida krusei is an opportunistic pathogen commonly implicated in urinary tract infections in immunocompromised patients. We present the first case of C. krusei renal cyst infection, occurring in a post-liver and kidney transplant patient with autosomal dominant polycystic kidney disease. Her persistent candiduria and fevers were refractory to prolonged therapy with AmBisome (Fujisawa Pharmaceuticals Co. Ltd., Osaka, japan). She eventually required bilateral nephrectomies of her native kidneys. Cystic fluid was aspirated from six hemorrhagic and six nonhemorrhagic cysts. Cystic fluid cultures yielded C. krusei. Fluid from the nonhemorrhagic cysts was also analyzed for amphotericin b levels, measured using a bioassay. Free amphotericin b levels in the cysts were lower than the minimal inhibitory concentration for amphotericin b for this organism. Urinary ascites and anuria caused by bilateral fungal balls in a premature infant.A case is reported of anuria and urinary ascites secondary to bilateral ureteropelvic obstruction by fungal balls. Management consisted of bilateral nephrostomy drainage with local irrigation with amphotericin b, and systemic antifungal treatment without surgery. Aspiration by paracentesis was performed for the urinary ascites and continuous drainage through an 8 Fr pig tail catheter for the urinoma. 8/29. Nephrogenic adenoma arising in a renal cortical cyst.Nephrogenic adenoma is an unusual lesion of the urinary tract first described by Davis in 1949 as a hamartoma. In 1950, Friedman and Kuhlenbeck subsequently characterized the lesion in more detail and named it nephrogenic adenoma. It appears to be a metaplastic response of the urothelium elicited by trauma and chronic irritation and is considered to be benign. A medline search of the literature from 1960 to the present revealed no previous documentation of a nephrogenic adenoma arising from a renal cortical cyst.
