1/3. Small bowel intussusception due to metastatic malignant melanoma. A case report.

    Small bowel metastatic deposits attributed to malignant melanoma are found in 2-5% of patients with malignant melanoma of the skin. Ileo-ileo intussusception caused by metastatic melanoma is a very rare condition. The prognosis of metastatic melanoma is poor. We report a case of a cutaneous malignant melanoma which metastasised to the small bowel causing enteroenteric intussusception. This case refers to a 66-year-old male patient who underwent surgery for suspected enteric intussusception. This diagnosis was suggested by computer tomography scan. The patient had had previous surgery for a primary malignant melanoma in the eyelid of the right eye. Segmental intestinal resection with regional lymph node dissection and ileo-ileo anastomosis was performed. Metastatic melanoma in the gastrointestinal tract should be suspected in patients with history of melanoma of the skin and acute gastrointestinal symptoms. Immediate laparotomy and excision of the affected bowel segment is the appropriate treatment.

2/3. abdominal pain with rigidity secondary to the anti-emetic drug metoclopramide.

    We report a case of abdominal pain with rigidity, mimicking an acute abdomen, caused by metoclopramide, a common anti-emetic drug. Extrapyramidal symptoms are commonly reported side-effects of this medication. They generally include involuntary movements of limbs, torticollis, oculogyric crisis, rhythmic protrusion of tongue, trismus, or dystonic reactions resembling tetanus, etc. Abdominal rigidity due to this medication, resembling an acute abdomen, has not been reported previously. This case report illustrates the importance of considering medication side-effects when evaluating a patient with abdominal pain and rigidity.

3/3. Ultrasonographic demonstration of intussusception in the acute abdomen in a Zambian infant.

    ultrasonography demonstrated intussusception in a healthy and well nourished seven-month-old infant who presented with a four-day history of vomiting and abdominal pain. Plain film showed a right-sided abdominal mass, a paucity of gas within the right lower quadrant and loops of distended small bowel. This led to ultrasonography of the gastrointestinal tract that demonstrated typical "bull's eye" pattern within the intussusception process. No barium studies were performed and confirmation was obtained at operation. ultrasonography of the bowel may be of value when investigating children with atypical acute abdomen.
