1/14. Metastases to the products of conception from a maternal bronchial carcinoma. A case report and review of literature.

    The association of mother's malignancy with metastases to the products of conception is rarely reported. We present 8th case reported in the literature of oat-cell lung carcinoma complicated by an aggressive clinical course and documented placental metastases. review of all published cases of lung cancer metastasizing to the placenta is also presented. A 44-year-old white pregnant woman at an estimated gestational age of 30 weeks presented with cervical incompetence symptoms and multiple tumors of 2-4 cm in diameter located in the subcutaneous tissue of the chest, including both breasts. The biopsy revealed a metastatic oat-cell carcinoma. cesarean section was performed immediately. During the procedure, micronodular metastatic infiltration of the liver was noted. Microscopic examination of placenta showed clusters of oat-cell carcinoma calls within the intervillous spaces; no infiltration of villi themselves was noted. The malignant cells were morphologically identical as those of the subcutaneous metastases. The woman succumbed to cancer and died on the 16th postoperative day. lung cancer metastasizing to the placenta is relatively rarely reported. In all cases only placental metastases were found; the fetus in all cases was spared. Only in one out of 8 reported cases outcome of mother was not fatal. It should be emphasized that not in all cases of maternal malignancy the histological examination of the placental was made Therefore it is possible that the incidence of placental involvement is more often than it is reported.

2/14. Intra-uterine death resulting from placental metastases in adenocarcinoma of unknown primary.

    A thirty-five year old woman presented with bilateral neck, chest wall and back masses. She was 16 weeks pregnant. lymph node excision revealed metastatic poorly differentiated adenocarcinoma of unknown primary. Abdominal ultrasound showed a mildly enlarged spleen and a 2-3 cm porta hepatis node. All other investigations were negative. The lymph node and cutaneous metastases progressed rapidly so it was decided to initiate systemic chemotherapy with a view to delivery at 28 weeks gestation by Caesarean section. Shortly after the second 3-weekly cycle of cisplatinum chemotherapy the patient suffered severe lower back and hip pain with MRI scan showing multiple bony metastases in the pelvic girdle. Ultrasound revealed the fetus to have been dead for at least 10 days. The products of conception were delivered following medical induction of labour. Two days later the patient suffered a cardiac arrest from which she could not be resuscitated. Placental histology revealed extensive metastases. With the exception of melanoma this has rarely been reported in solid adult malignancy. As a cause of fetal death, placental metastases are extremely rare.

3/14. lung cancer during pregnancy involving the products of conception and a review of the literature.

    Approximately 1 per 1,000-1,500 pregnancies is complicated by maternal malignancies. Metastatic involvement of the products of conception is a rare event. There have been 62 cases of placental and/or fetal metastatic involvement originating from maternal cancer reported since 1866. Only 14 cases of lung cancer associated with pregnancy have been documented. We report on an additional case involving the products of conception, and the management of lung cancer in pregnancy is discussed based on an extensive review of the literature. The case of a 29-year-old woman presenting during the 31(st) week of gestation with metastatic non small-cell lung cancer to the placenta, liver and bone is described. The mother was delivered by caesarean section of a healthy baby girl during her 32(nd) week of gestation. The mother's postpartum course was complicated by disseminated pulmonary and bony metastases and malignant pericardial and pleural effusions causing the patient's death within 1 month after lung cancer was diagnosed. Malignancies spreading to the products of conception are melanoma (32%), leukemia and lymphomas (15%), breast cancer (13%), lung cancer (11%), sarcoma (8%), gastric cancer (3%) and gynecologic cancers (3%), reflecting malignancies with a high incidence in women of reproductive age. All lung cancers were diagnosed with widely disseminated, inoperable neoplastic disease, including distant metastases in 46%. The mean age was 35.1 years (range, 30-45 years) and 60% of patients had a history of tobacco use. The mean survival was 7.5 months (range: 1-42 months). placenta involvement was present in 7 out of 15 cases. Fetal involvement was reported in only one case. Because there is no evidence of a direct adverse effect of pregnancy on the course of lung cancer, we recommend delivery at a time when enough fetal maturity can be assumed and the subsequent treatment of the mother.

4/14. Ovine chlamydiosis in an abattoir worker.

    The strain of chlamydia psittaci causing enzootic abortion in ewes (the EAE strain) may cause serious infection in pregnant women, often resulting in hepatic and renal dysfunction, disseminated intravascular coagulation and fetal loss. The first case of such an infection in an abattoir worker is described and the possibility of human-to-human transmission considered. Direct handling of sheep or their products of conception can usually be established but this is not always so. There is much still to be learned about this uncommon but severe zoonosis.

5/14. Maternal malignancy metastatic to the products of conception: a review.

    Documented reports of maternal malignancy metastatic to the placenta and fetus are rare. From 1866 until the present there have been 52 cases reported in the Western literature. We report a case of maternal large-cell carcinoma of the lung metastatic to the maternal brain and the placenta without fetal involvement.

6/14. The antenatal diagnosis of hyperplacentosis, hydramnios and fetal ascites.

    The diagnostic value of volumetric analysis of the products of conception by compound ultrasonic scanning is illustrated by a case of hyperplacentosis, hydramnios, and fetal ascites. The use of this method in the diagnosis of fetal conditions is discussed.

7/14. Placental metastasis from maternal carcinoma of the lung.

    A case of large cell undifferentiated carcinoma of the lung metastatic to the placenta and a review of the literature are presented. This is the 44th documented case of placental and/or fetal metastasis from maternal cancer in the past 113 years. Fifty percent of these cases are either malignant melanomas or hematopoietic malignancies. Evidence indicates that these 2 malignancies are more likely to metastasize to the products of conception than are other malignancies.

8/14. Placental metastasis from maternal carcinoma. A report of three cases.

    Metastasis of a maternal neoplasm to the products of conception is extremely rare. Of the 54 reported cases in the world literature, only 14 (25%) showed fetal metastasis. More than 50% of the reported cases were not examined grossly or had no visually apparent tumor deposits. Malignant melanoma is the most common malignant maternal neoplasm to metastasize to the products of conception. We report three unusual maternal malignant neoplasms (one pancreatic and two breast cancer) with evidence of placental metastasis and discuss the risk factors for fetal involvement in these cases.

9/14. Orbital rhabdomyosarcoma metastatic to the placenta.

    Cancer coexisting with pregnancy is well reported; however, maternal malignancies that are metastatic to the products of conception are rare. We report a case of orbital rhabdomyosarcoma that metastasized to the placenta.

10/14. Maternal malignancy involving the products of conception: a report of malignant melanoma and medulloblastoma.

    PURPOSE: Cancer in pregnancy is not uncommon, but cases of maternal malignancy involving the placenta or the fetus are much more rare. We report two additional cases. patients AND methods: We report one case of malignant melanoma discovered during pregnancy and found to have metastasized to the placenta. We also describe a case of maternal medulloblastoma involving the placenta at delivery. CONCLUSIONS: Although maternal malignancy during pregnancy occurs in as many as one in 1,000 pregnancies, involvement of the products of conception is rare. Subsequent malignancy in the fetus is even more rare. We report two cases, one of melanoma and another of medulloblastoma (the first such case described). Both infants are alive and well.
