11/25. Prolapsing large aneurysm of the atrial septum simulating a right atrial mass.

    An unusual case of a large, prolapsing atrial septal aneurysm in a patient with an otherwise normal heart is described. The aneurysm caused right atrial obstruction and resulted in a "tumor effect." The patient experienced debilitating symptoms for years before receiving an appropriate diagnosis and curative surgical treatment.

12/25. colostomy prolapse and hernia following window colostomy in congenital pouch colon.

    Congenital pouch colon, a variant of anorectal malformation, is a rare anomaly with a high incidence in North india and although the anatomy and diagnostic features have been well described, the surgical treatment continues to be challenging. This report describes the complications following a less preferred but often practised surgical option-a temporizing window colostomy in three babies. A window colostomy predisposes to prolapse of the entire pouch colon with herniation of bowel loops, intestinal obstruction and failure to thrive. Depending on the state of the child, either a proximal diverting stoma or tubularization of the pouch with a terminal stoma is recommended as the procedure of choice for the initial surgical management of this complex and rare malformation.

13/25. Sudden asphyxial death due to a prolapsed esophageal fibrolipoma.

    Fibrolipomas of the esophagus are extremely uncommon benign tumors. Accurate diagnosis and resection are essential, due to their tendency to become impacted and obstruct the airway. The case is presented of a 56-year-old man who died suddenly of asphyxia because of upper airway obstruction by a prolapsed fibrolipoma of the esophagus.

14/25. Prolapsed ureterocele.

    Although rare, the prolapsed ureterocele constitutes a urologic emergency that can rapidly evolve to severe conditions due to congestion and necrosis, and to obstruction of the urinary tract. A simple external resection resolves the emergency, allowing a postoperative investigation of the whole urinary tract to be performed, in order to adequately prepare urinary reconstruction.

15/25. The Andy Gump fracture of the mandible: a cause of respiratory obstruction or distress.

    Bilateral body fractures of the elderly edentulous atrophic mandible can cause a characteristic facial deformity and prolapse of the tongue into the pharynx, obstructing the airway (3). Early clinical and radiographic diagnosis of this injury is important. Stabilization of the fracture fragments restores the bony support for the tongue and helps establish patency of the airway.

18/25. adult floppy epiglottis: a simple surgical remedy.

    Epiglottic prolapse during inspiration is an unusual cause of upper airway obstruction. We present a case of a previously fit, 73-year-old, male with upper airway obstruction secondary to a floppy epiglottis. We describe a simple method of treatment without the need for partial or total excision of the epiglottis or tracheostomy.

19/25. Epiphora secondary to punctal apposition in the setting of Graves' orbitopathy.

    The authors describe a patient who had epiphora and lacrimal outflow obstruction associated with proptosis and prolapse of orbital fat into the eyelids. The patient's puncta remained opposed throughout the blink cycle. A bilateral transcutaneous orbital fat decompression was done via upper eyelid crease and lower eyelid infraciliary incisions. Significant extraconal fat was removed. Two months after surgery, the epiphora had completely resolved, and proptosis and eyelid fullness had decreased.

20/25. Prolapsed vesicostomy results in a strangulated bowel herniation: a rare complication of cutaneous vesicostomy.

    A 3-year-old girl presented with a prolapsed cutaneous vesicostomy, and symptoms and signs of acute intestinal obstruction. The vesicostomy had been performed when she was 19 months old for bilateral vesicoureteral reflux. Postoperatively, prolapse of the bladder from the stoma occurred almost daily. The prolapsed part retracted naturally when the patient was relaxed and reclining. intestinal obstruction was due to strangulated herniation of the bowel into the prolapsed bladder via the stoma of vesicostomy. Treatment consisted of manual reduction and the stoma of the vesicostomy was revised with the patient under general anesthesia.
