1/21. Management of prostatic fistulas.

    Prostatic fistulas communicating with the rectum or perineal skin are unusual complications of a prostatic operation, pelvic trauma, prostatic abscess or other iatrogenic injury. A third of these fistulas may close spontaneously with proper urinary drainage and avoidance of fecal soilage. The many operative procedures described for the repair of these fistulas indicate that no ideal method of repair can be applied to every case. Operative management should be mandated by the size, location and duration of the fistula as well as by the surgeon's experience with the various anatomic approaches.

2/21. Prostatorectal fistula associated with tubular colorectal duplication.

    Tubular colonic duplications are exceedingly rare. The authors present an unusual case of a boy with a persistent prostatorectal fistula resulting from a tubular colorectal duplication. The current case is unique for 2 reasons: (1) the presence of a fistula without any concomitant genitourinary anomalies and (2) the existence of a prostatorectal fistula.

3/21. A case of rectoprostatic fistula due to prostatic abscess visualized by barium enema.

    We report a rare case of rectoprostatic fistula due to spontaneous rupture of a prostatic abscess in a patient with diabetes mellitus. barium enema clearly showed rectoprostatic fistula. barium enema and colonoscopy were very useful in demonstrating the rectoprostatic fistula.

4/21. Successful closure of a recto-prostatic fistula.

    Recto-prostatic fistula is a rare complication of prostatic surgery, occurring usually because surgical planes are not appreciated. We describe a combined abdomino-perineal approach for the repair of a large recto-prostatic fistula with the interposition of omentum and gracilis without formally closing the fistula in layers.

5/21. polytetrafluoroethylene and autologous fat endoscopic injection for a prostato-perineal fistula.

    PURPOSE: Prostato-perineal fistula is a rare abnormality. We present a simple endoscopic technique to manage this complicated problem. MATERIALS AND methods: We report on a 34-year-old man with a prostato-perineal fistula after multiple surgeries for imperforate anus. The patient underwent an endoscopic injection of polytetrafluoroethylene and autologous fat in the fistulous tract. RESULTS: Two endoscopic procedures with the injection of polytetrafluoroethylene and autologous fatty tissue around the prostatic urethral orifice were necessary to occlude the fistula. CONCLUSIONS: The endoscopic injection of polytetrafluoroethylene and autologous fat was an easy, well tolerated and long lasting solution in this patient.

6/21. Conservative treatment of iatrogenic urinary fistulas: the value of cyanoacrylic glue.

    Once previously attempted conservative maneuvers have failed, iatrogenic persistent urinary fistulas usually require difficult repeated operations. We describe 3 patients in whom cyanoacrylic glue was used to repair endoscopically persistent urinary fistulas occurring after major pelvic surgery. At a mean follow-up of 21 months, all patients were free of urinary leakage and had no evidence of recurrent urinary fistulas. This approach may represent a safe and effective way to repair postoperative urinary fistulas.

7/21. Robot-assisted Bricker ileoureteral anastomosis during intracorporeal laparoscopic ileal conduit urinary diversion for prostatocutaneous fistula: case report.

    BACKGROUND AND PURPOSE: The da Vinci robot is useful during minimally invasive surgery in performing intracorporeal suturing. We report one case of its application during laparoscopic ileal conduit urinary diversion for prostatocutaneous fistula. methods: A 58-year-old paraplegic man with a neurogenic bladder and bowel and a long history of urinary incontinence developed a prostatocutaneous fistula after numerous procedures to correct the incontinence. He underwent laparoscopic ileal conduit urinary diversion to improve his quality of life. The da Vinci robot was used to perform the ileoureteral anastomosis. RESULTS: The operative time was 10 hours. The estimated blood loss was <100 mL. There were no intraoperative complications. The patient was started on a clear liquid diet on postoperative day 3. There was no narcotic use because of the patient's neurologic status. The patient was discharged home on day 6. CONCLUSION: Laparoscopic urinary diversion remains a technically challenging procedure. The da Vinci robot is useful during laparoscopic ileal conduit construction.

8/21. Delayed post-traumatic prostatic-urethrorectal fistula: transperineal rectal sparing repair - point of technique.

    We describe the outcome and management of an unusual and interesting case of delayed post-traumatic prostatorectal fistula in a 40-year-old man. The fistula was repaired successfully via transperineal access without rectal or sphincteric transgression. We found the transperineal surgical approach simple, effective and useful in approaching the prostatorectal region for rectourinary fistulas. The transperineal approach is useful and should be considered in such select cases. We describe our technique that may be beneficial to many urologists.

9/21. diagnosis and conservative treatment of tubercular rectoprostatic fistula.

    OBJECTIVE: To present our experience with three cases of rectoprostatic fistula with special emphasis on diagnosis and conservative management.patients AND methods: Three middle-aged men presented to us differently. All had spontaneous rectoprostatic fistulas. biopsy showed tuberculosis though three consecutive urine samples for acid-fast bacilli were negative. None of the patients were immunocompromised. Their upper tracts were normal and all had a past history of pulmonary tuberculosis. They were started on antitubercular drugs and urinary diversion with or without faecal diversion. RESULTS: All fistulae healed completely within 6 weeks of starting antitubercular treatment. One patient healed with bladder neck stenosis that required bladder neck incision. Voiding was normal on 1-year follow-up. CONCLUSION: Spontaneous tubercular rectoprostatic fistulae are rare. There should be a strong clinical suspicion in endemic areas. Prostatic biopsy proves the diagnosis. Conservative management with antitubercular drugs and urinary diversion with or without faecal diversion has a high success rate and should be the first line of treatment even if urine is negative for acid-fast bacilli.

10/21. Giant prostatic urethral calculus associated with urethrocutaneous fistula.

    Urethral stones in men are rare clinical entity and most of them migrate from the urinary bladder. Urethral stones are rarely formed primarily in the urethra and are usually associated with urethral strictures or diverticula. We report a 41-year-old man with giant prostatic urethral stone (5.9x3.2x2.8 cm) associated with a urethrocutaneous fistula. The etiological factors, pathogenesis, clinical presentation, complications and management of giant urethral calculi are reviewed.
