Nonoperative management of iatrogenic prostatorectal fistula.Iatrogenic prostatorectal fistula is a rarely seen urologic complication of transurethral resection of the prostate and stricture dilatation with sounds. In the past, such cases have been managed with colostomy diversion of the fecal stream and suprapubic cystostomy diversion of the urinary stream and in most cases operative closure of the fistula. This case illustrates that a more conservative nonoperative approach can lead to successful closure of such a fistula in selected cases. Prostatic and membranous urethrorectal fistulas: a new technique of surgical closure.We report on a quick, safe and simple procedure, the anterior perineal trans-anorectal approach, which was used successfully to treat 2 difficult cases of prostatorectal and membranous urethrorectal fistulas. This method was devised after a review of all techniques to treat these fistulas, which are known to be difficult. Our 2 cases, like others, demonstrate the harmlessness of anterior section of the anal sphincter, provided the sphincter is repaired correctly. 17/21. Surgical correction of rectoprostatic fistula.A rectoprostatic fistula is a rare complication of prostatic operations, and the literature on the subject is scanty. Three men were treated for rectoprostatic fistulas representing complications of prostatic surgery. Mason's transsphincteric transrectal operation resulted in prompt, primary healing of the fistula. Subsequent colostomy closure left all 3 patients with normal urinary and fecal control. A systematic three-step sequence of operation is advocated for the treatment of rectoprostatic fistulas. 18/21. Transvesical approach to utricular cysts.Utricular cyst is an uncommon problem, mainly encountered in intersex conditions and is usually asymptomatic. These lesions have been classified in three main groups following the degree of masculinization by Howard. Three of our patients nevertheless have presented with very severe utricular and urinary infections, one of these having developed a scrotal fistula. Another was incontinent due to intra-utricular trapping. The first patient was operated upon by a perineal approach which we found extremely difficult.
