1/5. Use of tunica vaginalis patch graft for repair of traumatic testicular rupture.

    Early exploration of patients with testicular rupture has become the standard of care in traumatic testicular injuries. We report on a case of traumatic rupture of the testicle diagnosed by ultrasonography, with a large testicular defect that could not be closed primarily. This was managed successfully with a free graft of tunica vaginalis. The affected testicle is palpably normal three months postoperatively. This technique may represent a valuable adjunct in managing major testicular ruptures.

2/5. color Doppler sonographic findings in penile fracture.

    We performed color Doppler sonographic examination on 4 patients, 3 of whom had preliminary diagnoses of penile fracture and 1 of whom had undergone an operation due to penile fracture a year previously. color Doppler sonography helped evaluate the relationships between the hematoma and the vascular structures and aided in differential diagnosis of the vascular injuries that may accompany tunical rupture (or have similar clinical presentations) and Mondor's disease; thus, this method has a crucial role in choosing the treatment approach. color Doppler sonography may also assist in the follow-up of patients after surgical or conservative treatment.

3/5. Fractured penis: a clinical misnomer!

    BACKGROUND: A 'fracture' of the penis refers to a tear in the deep layer of the tunica albuginea (Buck's fascia) and may be associated with a urethral injury. AIM: To describe a classical case of a 'fractured' penis and discuss the management options. METHOD: A case report of a 30-year-old man presented with a 'fractured' penis and who underwent surgical intervention. CONCLUSION: This rare occurrence represents a urological emergency and necessitates imaging and repair of the cavernosal defect in order to prevent poor functional outcome.

4/5. Bilateral testicular rupture: report of a case with an unusual presentation.

    A case of bilateral testicular rupture after blunt trauma is presented. It is unusual in that the patient presented with a unilateral retractile testis with pain. The contralateral testis was asymptomatic but was found to be ruptured at exploration. The objectives of preservation of testicular function and prevention of misdiagnosis and undiagnosed cases are reiterated. Early exploration of all blunt trauma to the scrotum and testis is advocated. The surgical management of the ruptured testis is reviewed with regard to orchiectomy, primary closure, closure with portion of tunica vaginalis and decompression of the contused testis.

5/5. Injury of the tunica media in fatal rupture of the vertebral artery.

    The histologic findings in seven cases of traumatic basal subarachnoid hemorrhage from rupture of a vertebral artery are reported. In all cases, blows to the head or neck resulted in minor injury to the cranial or cervical soft tissues but the ipsilateral intracranial segment of the vertebral artery was the site of a longitudinal transmural tear. In addition to rupture, the arterial wall proximal and distal to the site of rupture showed evidence of distortional injury of the tunica media. These traumatic lesions appeared as patchy anuclear regions of the media with distinctive pallor of smooth muscle staining. We propose that the forces which cause vertebral artery rupture stretch the artery along its long axis, thereby distorting the artery leading to this characteristic histologic change.
