1/85. Central line sepsis in a child due to a previously unidentified mycobacterium.

    A rapidly growing mycobacterium similar to strains in the present mycobacterium fortuitum complex (M. fortuitum, M. peregrinum, and M. fortuitum third biovariant complex [sorbitol positive and sorbitol negative]) was isolated from a surgically placed central venous catheter tip and three cultures of blood from a 2-year-old child diagnosed with metastatic hepatoblastoma. The organism's unique phenotypic profile and ribotype patterns differed from those of the type and reference strains of the M. fortuitum complex and indicate that this organism may represent a new pathogenic taxon.

2/85. Endogenous endophthalmitis associated with bacillus cereus bacteremia in a cocaine addict.

    A 22-year-old black female intravenous cocaine addict presented with an endophthalmitis of the right eye. Diagnostic evaluation included an immediate anterior chamber paracentesis and a delayed vitreous aspiration. Although cultures from the involved eye were negative, all 7 blood cultures grew Bacillus cereus suggesting that this organism was the responsible agent of an endogenous endophthalmitis. The patient was treated with appropriate systemic and local antibiotics with resolution of the acute inflammatory signs. However, a phthisical eye has been noted on follow-up examinations.

3/85. Immune deposit nephritis in infectious mononucleosis.

    A 22-year-old white male (L.V.) died of gram-negative septicemia complicating infectious mononucleosis (IM) that was associated with jaundice and oliguric renal failure. The kidney showed mesangial granular deposits of IgM and C3, mesangial electrondense deposits, and interstitial infiltrates of infiltrates of mononuclear cells, including atypical lymphocytes. Eluates obtained from kidney, spleen and liver contained Paul--Bunnell (PB) antibodies. Presence of PB antigens in these tissues was indicated by absorption of PB antibodies from IM sera, with the sediments resulting from tissue elutions. The IgM mesangial deposits were partially eluted with acid buffer at 56 degrees C and then reconstituted by incubation with IM sera or with immunoglobulins eluted from tissues of patient L.V. The presence in renal structures of PB antigens, IgM heterophile antibody, C3 and electron-dense deposits is consistent with the hypothesis that heterophile immune complexes were localized in the kidney and that they contribute in the pathogenisis of IM nephritis.

4/85. Thoracic complications of lemierre syndrome.

    lemierre syndrome is a severe, septicemic illness most commonly caused by the anaerobic Gram-negative bacillus fusobacterium necrophorum. It is characterized by an acute oropharyngeal infection, with secondary septic thrombophlebitis of the internal jugular vein and frequent metastatic infections. This report of a patient with the lemierre syndrome is complemented by a review of the literature on the subject.

5/85. Severe disseminated BCG infection in an 8-year-old girl.

    An 8-year-old girl died of sepsis due to staphylococcal infection one year and 8 months after Bacille Calmette-Guerin (BCG) revaccination. Two months after the vaccination in accordance with the school health program, she was hospitalized with a high fever, skin rash over the face and lower limbs, and leukopenia. Her clinical and laboratory pictures were not compatible with those of any established type of immunodeficiency. The polymerase chain reaction (PCR) test for M. tuberculosis complex was positive for bone marrow, pleural fluid, and peripheral blood. The strain recovered from a mycobacterial culture of the blood was identical to the BCG strains with which the patient was vaccinated, based on restriction fragment length polymorphism (RFLP) and a pulse-field gel electrophoresis (PFGE) analyses of dna. She developed finally a lung abscess due to staphylococcal septicemia, which was the direct cause of her death.

6/85. Enterobacter sakazakii infection in the newborn.

    Enterobacter sakazakii, a Gram-negative bacillus, previously known as "yellow pigmented enterobacter cloacae", is a rare cause of neonatal infection. We describe the detailed clinical presentation of two cases in whom E. sakazakii was isolated in our neonatal service during the course of 1 mo. These include one case of sepsis and meningitis complicated by cerebral infarction, and one case of sepsis. In addition, three cases of intestinal colonization were identified. The source of the organism was thoroughly sought and was found to be a blender in the milk kitchen that was used for preparation of the reconstituted powdered milk formula. CONCLUSION: Our paper adds clinical and laboratory information about the disease spectrum caused by this relatively rare organism and emphasizes the importance of a thorough search for the source of the infection.

7/85. lactobacillus casei subsp. rhamnosus sepsis in a patient with ulcerative colitis.

    Lactobacillus species are part of normal gastrointestinal flora and are an uncommon cause of bacteremia. lactobacillus casei subsp. rhamnosus was repeatedly isolated in the blood cultures of a 43-year-old woman with ulcerative colitis. Lactobacillus spp. could be considered a potential opportunistic pathogen whenever there is disruption of the physiologic architecture of the intestinal mucosa, such as in abdominal surgery, endoscopic procedures, or enteric pathologies. Clinicians and clinical microbiologists need to be aware of this infective complication in patients with ulcerative colitis.

8/85. lactobacillus acidophilus sepsis in a neonate.

    Lactobacillus species are non-spore-forming, anaerobic, gram-positive rods that cause disease in immunocompromised adults. Few cases have been described in children. We present the case of a 2-month-old infant who apparently developed lactobacillus acidophilus sepsis from an infected central venous catheter. physicians should be aware that although Lactobacillus species rarely cause disease in children, they should be considered a possible pathogen when isolated from the blood of a newborn infant.

9/85. lactobacillus rhamnosus septicaemia in a patient with a graft in the inferior vena cava.

    A case of septicaemia that repeatedly showed growth of lactobacillus rhamnosus in blood cultures is reported. The patient improved after removal of a Goretex patch in the inferior vena cava that was the focus of the infection. Lactobacilli can, in certain settings, be the cause of clinically important infections.

10/85. bacteremia due to Succinivibrio dextrinosolvens. Report of a case.

    An unusual case report of a patient with bacteremia due to Succinivibrio dextrinosolvens is presented. Heretofore, Succinivibrio spp. were thought to occur only as organisms in the rumens of herbivorous animals. Succinivibrio is an anaerobic, Gram-negative, curved, spiral bacillus with a polar monotrichous flagellar pattern. Volatile acids produced from glucose metabolism include succinic, acetic, formic and lactic. Various carbohydrate substrates are fermented to produce strong or weak acid end-points. Succinivibrio spp. will grow in bile, will not hydrolyze esculin or starch, and do not produce indol, catalase, lecithinase, lipase, or hemolysis. bacteremia in this case was thought to have been the result of hypotension in the course of severe gastrointestinal hemorrhage. This is thought to represent the first published case of human infection due to Succinivibrio spp.
