1/17. Post-traumatic pituitary apoplexy--two case reports.

    A 60-year-old female and a 66-year-old male presented with post-traumatic pituitary apoplexy associated with clinically asymptomatic pituitary macroadenoma manifesting as severe visual disturbance that had not developed immediately after the head injury. skull radiography showed a unilateral linear occipital fracture. magnetic resonance imaging revealed pituitary tumor with dumbbell-shaped suprasellar extension and fresh intratumoral hemorrhage. Transsphenoidal surgery was performed in the first patient, and the visual disturbance subsided. decompressive craniectomy was performed in the second patient to treat brain contusion and part of the tumor was removed to decompress the optic nerves. The mechanism of post-traumatic pituitary apoplexy may occur as follows. The intrasellar part of the tumor is fixed by the bony structure forming the sella, and the suprasellar part is free to move, so a rotational force acting on the occipital region on one side will create a shearing strain between the intra- and suprasellar part of the tumor, resulting in pituitary apoplexy. Recovery of visual function, no matter how severely impaired, can be expected if an emergency operation is performed to decompress the optic nerves. Transsphenoidal surgery is the most advantageous procedure, as even partial removal of the tumor may be adequate to decompress the optic nerves in the acute stage. Staged transsphenoidal surgery is indicated to achieve total removal later.

2/17. Pituitary insufficiency after penetrating injury to the sella turcica.

    We report a 28-year-old male patient with a pituitary insufficiency after a simple fracture of the sella turcica. He was injured by a long nail that punctured the lower jaw. No fracture other than that of the sella turcica was detected. An endocrinological examination revealed both anterior and pituitary dysfunction and diabetes insipidus that continued for about two months.

3/17. Fracture of the sella turcica.

    A patient with a fracture of the sella turcica, visible on lateral X-ray films of the skull, is described. This fracture, although not diagnosed during life, was present in approximately 20 per cent of a series of consecutive autopsies on patients who died of head injury. The significance of this injury to the hypothalamopituitary axis is discussed and methods of investigation suggested.

4/17. Traumatic bilateral abducent nerve palsies.

    A patient sustained a severe cranio-facial injury which included a transverse fracture of the middle cranial fossa through the sella turcica producing otorrhoea, rhinorrhoea, a bilateral abducents palsy and a large aero-coele. All gradually remitted spontaneously. The management of this patient and the patterns of cranial base fractures and their associated clinical features, particularly of the middle cranial fossa, are discussed.

5/17. Posttraumatic diabetes insipidus.

    The authors report seven patients with fractures of the sella turcica. Four of them developed diabetes insipidus as a consequence of their traumata. The importance of computed tomography as a diagnostic procedure is paramount in order to make early diagnosis of this potential life-threatening condition possible.

6/17. Fracture of the sella turcica: a report of three cases.

    Fractures of the sella turcica are rare. Recognition of fractures in this area is very important as serious immediate and delayed neurological, endocrine and vascular complications may occur. Most of these patients will develop at least one cranial nerve palsy; a significant number will develop hypopituitarism and a few will develop local vascular abnormalities.

7/17. diplopia and diabetes insipidus secondary to type II fracture of the sella turcica: case report.

    Fractures of the sella turcica are rare and are often difficult to diagnose radiologically. They can produce a wide variety of complications, including septic, neuroendocrine, neurovascular and neuroophthalmologic problems. The authors describe the case of a 17-year-old boy with diabetes insipidus and diplopia secondary to a type II fracture of the sella turcica. They emphasize the clinical importance of fractures in the sellar region in view of their proximity to vital structures and they discuss possible mechanisms of causation.

8/17. Suprasellar arachnoid cysts: an extension of the membrane of Liliequist.

    A case of suprasellar arachnoid cyst and hydrocephalus diagnosed by computed tomography (CT) and positive-contrast ventriculography is presented. Transfrontal exploration of the 3rd ventricle revealed findings consistent with the cyst being, in fact, a large forward and upward diverticulum of the arachnoidal membrane of Liliequist. The authors suggest that some of the other similarly located arachnoid cysts are extensions of this membrane imperforate by previous infection, hemorrhage, or maldevelopment.

9/17. Fractures of the sella turcica.

    Five recent cases of sella turcica fracture examined at the Medical Center Hospital of vermont are reported. These fractures are most commonly associated with frontal or maxillofacial trauma and are complicated by cranial nerve palsies, chiasmatic injury, and cerebrospinal fluid rhinorrhea and otorrhea. The mortality rate with these fractures is high because of the associated brain injury. The importance of a detailed endocrine evaluation to detect developing pituitary abnormalities is also emphasized. The accuracy of radiographic diagnosis of these fractures utilizing plain skull films, tomography, and computed axial tomography is reviewed. the importance of cerebral angiography to evaluate the intracavernous internal carotid arteries and the basilar artery is also emphasized, as is the theoretical basis of the pathological findings associated with these fractures.

10/17. Traumatic bitemporal hemianopia and fracture of the dorsum sella. Case report.

    Fracture of the dorsum sella is uncommon. A case of traumatic bitemporal hemianopia with a fronto-basal fracture and a fracture of the dorsum sella is reported.
