Behavioral management of sleep disturbances secondary to chronic pain.The clinical efficacy of a behavioral management program for treating insomnia secondary to chronic pain was evaluated within a multiple-baseline design across subjects. Treatment consisted of a combination of stimulus control and sleep restriction procedures. Daily sleep diaries and all-night polysomnographic (PSG) measures were used to document changes in sleep/wake patterns. The results showed that treatment was effective in improving sleep patterns in all three patients. A substantial decrease of time awake at night was obtained and this was reflected by reductions of sleep onset latency, wake time after sleep onset, and early morning awakenings. sleep improvements were well maintained at follow-ups and were also paralleled by improved mood states. 13/27. A treatment trial of delayed sleep phase syndrome with triazolam.triazolam causes a phase-shift of the circadian rhythm of locomotor activity in golden hamsters. We attempted to treat two patients with delayed sleep phase syndrome (DSPS) with triazolam. In one male patient we administered triazolam when the sleep phase began to delay after chronotherapy. triazolam combined with chronotherapy enabled him to normalize and stabilize his sleep-wake cycle. But in the other female patient despite normalization of her sleep-wake cycle, she became depressive and did not continue taking the drug. The use of indirect hypnotic suggestions for insomnia arising from generalized anxiety: a case report.This case report concerns an aggressive, independent, and financially successful businessman who suffered with insomnia within the context of generalized anxiety disorder. The hypnotherapy included indirect suggestions for the insomnia delivered through the vehicle of metaphorical stories designed as an indirect intervention for the generalized anxiety. The client's perception of the problem was clarified and respected, but the choice of intervention strategy was designed to indirectly suggest more global changes. 15/27. chronic pain as a reticular formation syndrome.Evidence was previously presented to support the thesis that chronic pain is activated by neuronal elements that make up the multisynaptic short axon core of the reticular system (Andy and Peeler 1985). The present thesis, that chronic pain is a reticular formation syndrome, is based on a retrospective analysis of four patients with chronic pain who were successfully treated with a lesion in the anterior thalamus and stimulation electrode implants in the posterior thalamus and pontomesencephalic brain stem. The reticular formation was the common underlying anatomic substrate at those three sites. In addition to chronic pain, all the patients had other symptoms attributable to other body organs and systems. The number and type of symptoms that made up the syndrome differed between patients. Symptoms making up the core of the syndrome were pain, anxiety, nervousness, insomnia, and depression. Experimental and clinical findings are briefly presented to demonstrate the various reticular formation sites, pragmatically considered "reticular functional systems," from which symptoms may arise. It is hypothesized that the symptoms are recruited by a low threshold "pain oscillator" that is generated at one reticular site and subsequently permeates the rest of the reticular system. 16/27. Behavioral side effects of triazolam in psychiatric inpatients: report of five cases.triazolam was administered to five psychiatric inpatients for a two-week period. This period was preceded by a one-week placebo baseline and followed by another week of placebo administration. All conditions were double blind. By the second week of active drug administration, psychopathology greatly intensified across all of the patients with the emergence of anxiety, memory impairment, confusion, paranoid ideation, and hallucinations. The drug-induced behavioral changes persisted during the initial withdrawal period, but then gradually subsided. 20/27. thioridazine withdrawal-induced behavioral deterioration treated with clonidine: two case reports.Neuroleptic withdrawal reactions have significant clinical and medicolegal implications for individuals with developmental disabilities. Behavioral deterioration following neuroleptic taper can represent (a) relapse of a mental illness, (b) and anticholinergic rebound reaction, (c) tardive akathisia, and (d) possibly a supersensitivity psychosis. Such reactions may preclude the discontinuation of neuroleptic drug therapy, even in the absence of a drug-responsive psychiatric illness. In this report, the case histories of two individuals who experienced a thioridazine (Mellaril) withdrawal-induced behavioral deterioration were presented. Both reactions were characterized by anxiety and insomnia, and the patients' symptoms were relieved by uncontrolled treatment with clonidine (Catapres) therapy.
