1/131. Radiographic findings in ovarian teratomas in children.

    Twenty-one infants and children with ovarian teratomas are reported. Calcification or ossification occurred in 68%, a higher figure than quoted in prior series. Teeth were recognized in 29% of these. A lucent fat-containing mass was appreciated in only two patients. In younger children the tumor was more frequently abdominal rather than pelvic. In two patients the tumor was malignant. A patient with an amputated freely mobile teratoma is reported in detail and the differential diagnosis of mobile abdominal calcifications discussed.

2/131. hypercalcemia accompanied by hypothalamic hypopituitarism, central diabetes inspidus and hyperthyroidism.

    We present here a case of prominent hypercalcemia accompanied by hypothalamic tumor and Graves' disease. A 24-year-old man with hypothalamic tumor showed hypopituitarism, central diabetes inspidus (DI) and hyperthyroidism. nausea, loss of thirst and appetite, and general fatigue were found with the unveiling of hypercalcemia and hypernatremia. parathyroid hormone (PTH) and 1alpha-dihydroxyvitamin D levels were suppressed with a normal range of PTH-related protein values. One-desamino-(8-D-arginine)-vasopressin (DDAVP) and half-saline administration normalized hypernatremia, while hypercalcemia was still sustained. Administration of cortisone acetate and thiamazole reduced the elevated serum Ca level. In the present case, concurrent hyperthyroidism was assumed to accelerate skeletal mobilization of calcium into the circulation. Hypocortisolism and central DI was also considered to contribute, to some extent, to the hypercalcemia through renal handling of Ca.

3/131. Recurrent Wilms' tumour or retroperitoneal teratoma? A challenging case.

    The clinical and pathological features of a left renal tumour and a subsequent retroperitoneal tumour in a 2-year-old boy are presented. The nephrectomy specimen showed typical features of a triphasic Wilms' tumour with focal heterologous elements and intralobar nephrogenic rests. The tumour was assessed as clinical stage III and post-operative chemotherapy and radiotherapy were administered. A retroperitoneal mass, detected following completion of postoperative therapy, was mainly made up of skeletal muscle and mature adipose tissue. Nests of epithelium resembling ameloblastic nests and a unique structure reminiscent of a developing tooth were present.

4/131. Squamous cell carcinoma arising in a mature teratoma with metastasis to the urinary bladder.

    We describe a patient with squamous cell carcinoma arising in a mature teratoma. Magnetic resonance (MR) images revealed a solid lobulated mass attached to the ovarian cyst containing a fat-fluid level. The solid component with extension into pelvic fat showed as hypointensity on T2-weighted MR images with good enhancement. A metastatic tumor to the urinary bladder was also demonstrated.

5/131. Transabdominal application of transvaginal transducer enhancing depiction of mature cystic teratoma at 34 weeks' gestation.

    We present an unusual case in which a 36-year-old patient was referred for consultation due to increasing upper left abdominal pain at 35 weeks' gestation. Transabdominal ultrasonography disclosed an appropriate-for-gestational-age singleton, vertex-presenting fetus with normal anatomy. An unclear, complex, semisolid, semicystic mass was noted in the upper right abdomen. Due to the close proximity of the adnexal mass to the patient's abdominal wall, a high-frequency transvaginal transducer was applied transabdominally. Unlike the unclear images generated at conventional transabdominal ultrasonography, this application depicted a discrete mass with multiple characteristics consisting of calcifications, hair and fatty tissue, considered diagnostic of a mature cystic ovarian teratoma. The patient delivered spontaneously at 39 weeks' gestation. At 6 weeks' postpartum an elective laparotomy left ovarian cystectomy was performed and a mature cystic teratoma confirmed by pathology examination.

6/131. Conversion of multiple solid testicular teratoma metastases to fatty and cystic liver masses following chemotherapy: CT evidence of "maturation".

    Testicular germ cell tumour metastases may undergo "retroconversion" to mature differentiated teratoma following chemotherapy or irradiation. We report a patient with testicular germ cell liver metastases in whom computed tomography (CT) scans following chemotherapy demonstrated a reduction in CT attenuation of the liver lesions to that of cystic and fatty density. This is believed to represent CT evidence of liver metastasis "retroconversion", which offers the potential for non-invasive monitoring of histological progression.

7/131. Dumb-bell-type teratoma in the lumbar spine.

    We report a case of a lumbar teratoma in a 50-year-old woman. The teratoma showed a dumb-bell-type expansion at the level of the left L3/4 foramen with massive erosion of the L3 vertebral body. MRI revealed inhomogeneous signal changes in the tumor, which were histologically compatible with a mixture of bone, muscle, fat, and cyst containing sebaceous material. Complete resection of the tumor and spinal arthrodesis with pedicle screw fixation was necessary to obtain stability of the affected spinal segment.

8/131. CT findings in post-operative subphrenic abscess with teratomatous inclusions.

    A 39-year-old woman presented with abdominal pain after tubal sterilization. CT showed a subphrenic abscess with fatty inclusions owing to laceration or rupture of a mature ovarian teratoma. Although subphrenic abscess is a well recognized post-operative complication, and ovarian teratomas are frequent, a teratomatous inclusion within a subphrenic abscess is a unique finding.

9/131. Cervical spine anomalies and tumors in Weaver syndrome.

    Weaver syndrome is an autosomal dominant disorder comprising accelerated growth rate and rapidly advancing skeletal maturation. Previous reports suggest that the phenotype in adults may be sufficiently subtle to make diagnosis difficult. Half brothers with classical childhood findings of Weaver syndrome and their father with minimal clinical findings showed cervical spine anomalies that likely represent a consistent radiographic finding in this disorder. One of the children represents the third occurrence of neoplasia in Weaver syndrome.

10/131. Unusual "floating balls" appearance of an ovarian cystic teratoma: sonographic and CT findings.

    Ovarian cystic teratomas are cystic fatty tumors that can be easily diagnosed by sonography and CT. We present a case of ovarian cystic teratoma with an unusual sonographic appearance of mobile, hyperechoic, intracystic fat balls; this finding correlated well with the appearance on CT.
