11/113. multiple endocrine neoplasia type 2b.

    We report a case of multiple endocrine neoplasia type 2b (MEN 2B) in a 30-year-old woman. There was no family history of MEN 2B in her family. dna testing was carried out and a point mutation was found in exon 16, codon 918 (ATG to ACG) in the RET proto-oncogene. The woman died of medullary thyroid carcinoma, 13 years after a total thyroidectomy.

12/113. Bazex syndrome.

    Bazex syndrome is a paraneoplastic acrokeratosis appearing mostly at the same time as the underlying neoplasm. We report a typical case with a squamous cell carcinoma of the tongue and classical cutaneous symptoms disappearing with the treatment of the tumour.

13/113. A case of multiple endocrine neoplasia type 2b undiagnosed for many years despite its typical phenotype.

    We report the case of a 24-yr-old man with a typical phenotype of multiple endocrine neoplasia type 2b (MEN 2B). The patient had previously undergone minor surgery to remove multiple tumors on the lip, but he had no further examinations. MEN 2B was suspected owing to characteristic multiple ganglioneuromatosis when the patient presented with a goiter associated with high levels of plasma calcitonin and CEA. Aspiration biopsy cytology revealed medullary thyroid carcinoma (MTC), and abdominal computed tomography and nuclear scanning with metaiodobenzylguanidine revealed bilateral adrenomedullary tumors. Adrenomedullary function tests showed high levels of serum and urinary fractionated catecholamines, and genetic analysis showed a point mutation in the codon 918 (M918T) of the RET gene. The patient was diagnosed with MEN 2B and underwent right adrenalectomy and total thyroidectomy. No distant metastasis of the MTC was noted although MEN 2B had remained undiagnosed since the ganglioneuromatosis was first noticed. MEN 2B is a rare hereditary disorder, but the occurrence of characteristic ganglioneuromatosis was quite helpful in making the diagnosis.

14/113. Papillary cystadenocarcinoma arising from minor salivary glands in the anterior portion of the tongue: a case report.

    Papillary cystadenocarcinoma is a rare malignant neoplasm of the salivary gland. We report a case of papillary cystadenocarcinoma arising from the minor salivary gland in the anterior portion of the tongue of a 72-year-old male patient with a history of adenocarcinoma of the colon and prostate. Further, we discussed histopathological and clinical features of this lesion, and reviewed the literature.

15/113. Target-specific multimodality endovascular management of carotid artery blow-out syndrome.

    We describe a novel multimodality endovascular approach to safely control hemorrhage from a carotid artery pseudoaneurysm and tumor vasculature associated with a squamous cell carcinoma. This approach was used in the case of a 68-year-old man who had previously undergone a laryngectomy, chemotherapy, and brachytherapy and who subsequently experienced acute oropharyngeal bleeding. angiography detected a carotid artery pseudoaneurysm and significant tumor vascularity. A target-specific multimodality approach was taken to embolize the potential etiologies for both the current and any future hemorrhages. Stent-assisted coiling of the pseudoaneurysm was successful. The tumor blush was treated with polyvinyl alcohol particles and both retrievable and nonretrievable coils. Endovascular surgeons have become increasingly involved in the management of patients with carotid injuries and with neoplasms in and around the skull base. Current endovascular technology provides a rapid target-specific approach to the treatment of carotid artery blow-out syndrome and has a greater potential to lower morbidity than does carotid sacrifice.

16/113. Solitary fibrous tumor of the tongue: report of a case with immunohistochemical and ultrastructural studies.

    solitary fibrous tumors are rare in extrapleural sites and extremely rare in the oral cavity. We report a case of a solitary fibrous tumor arising from the tongue of a 70-year-old woman. The tumor measured 1.6 cm in maximum diameter and consists of spindle-shaped cells distributed in a haphazard pattern. Immunohistochemical studies show strong positivity for CD34 and bcl-2, and weak positivity for desmin. Smooth muscle actin and S-100 protein are negative. Electron microscopy shows uniform neoplastic spindle cells with mesenchymal features. The differential diagnosis for spindle cell neoplasms in the tongue is discussed.

    BACKGROUND: Primary extramedullary plasmacytomas (PEMP) are rare malignant neoplasms with a predilection for the head and neck. Eighty percent of all PEMP are located in this area. CASE REPORT: The authors present a case of lingual plasmacytoma in a 65-year-old man, diagnosed on biopsy and treated with external beam radiotherapy.

18/113. Laparoscope-assisted distal gastrectomy for early gastric cancer in a 76-year-old man with situs inversus totalis.

    We report a case of a 76-year-old man with situs inversus totalis (SIT) who developed early gastric cancer. He was successfully treated by laparoscope-assisted distal gastrectomy for the gastric cancer. This case suggests that patients with SIT with malignancy could also undergo laparoscopic surgery. To the best of our knowledge, no case of laparoscopic surgery for malignant neoplasms associated with SIT has been reported in the English literature. We therefore believe this is the first case report.

19/113. Solitary fibrous tumour of the tongue: a case report with immunohistochemical studies.

    The solitary fibrous tumour (SFT), also known as a localized fibrous mesothelioma, is a neoplasm characterized by the proliferation of capillaries surrounded by masses of round or spindle-shaped cells. SFT has been discovered in many locations, however, it is extremely rare for SFT to be found in the intraoral region. We report here case of SFT arising from the tongue of a 75-year-old woman. To our knowledge, this is the second case reported of SFT originating from the tongue.

20/113. Non-functioning adrenal cortical carcinoma presenting with metastasis to the tongue.

    We report a case of non-functioning adrenal cortical carcinoma (ACC) presenting with metastatic disease to the tongue, which is an extremely uncommon onset for this neoplasm. Histologically, the lesion had the appearance of an anaplastic neoplasm, and a panel of immunohistochemical markers including vimentin, MART-1, S100 protein, HMB-45, smooth muscle actin, common muscle actin, desmin, CD31, CD34, CD68, EMA and cytokeratins, was helpful in excluding melanoma, as well as other mesenchymal and epithelial neoplasms.
