21/131. Repair of untreated horizontal root fractures: two case reports.

    Intra-alveolar root fractures of incisor teeth are more frequent than other dental injuries. These case reports describe two untreated horizontal root fractures of the maxillary right central incisors. patients reported accidental trauma, which occurred several years ago. These fractures were discovered during a routine full-mouth radiographic survey. Teeth were asymptomatic and tested vital to electric pulp tests.

22/131. Reattachment of a fractured maxillary incisor crown--case report.

    Management of traumatic injuries to the teeth is a challenge to the practising dentist. It has no prescriptive method for occurring, possesses no significant predictable pattern of intensity or extensiveness and occurring at times when dentists are least prepared for it. It may not only leave physical scars but also a psychological impact on its victim. Yet, more than half of all children traumatize either their primary or permanent teeth before leaving the school, coupled with the dynamic panorama of sporting activity worldwide and the significant increase in violence among the populations. tooth trauma and its management loom as a major challenge to the dental practitioner. A case of a 11 year old boy with Ellis Class 3 traumatic injury to the maxillary left central incisor is reported. A pulpectomy was performed followed by reinforcement and reattachment of the crown fragment and restoration with composite resin.

24/131. The combination of a surgical and adhesive restorative approach to treat a deep crown-root fracture: a case report.

    Most dental trauma requires immediate action to preserve the affected teeth. Furthermore, the trauma has often a great impact on the appearance and well being of the patient. In this case trauma resulted in a very deep complicated crown-root fracture of one incisor and a complicated crown fracture in another incisor of a 47-year-old woman. A combination of surgical extrusion and adhesive restoration was chosen to provide good prognosis for the teeth as well as an instant esthetic result that was well accepted by the patient. Eleven months after trauma, the treated teeth exhibited good healing and normal function.

25/131. Reattachment of an autogenous tooth fragment in a fracture with biologic width violation: a case report.

    An 11-year-old patient that fractured her maxillary left central incisor is presented. The fracture involved two thirds of the crown, compromising the pulp, and extended subgingivally on the palatal aspect, invading the biologic width. The procedure used to repair the fracture included flap surgery with a slight ostectomy and endodontic treatment. The reattachment of the tooth fragment and the restoration were performed with a bonding system and a resin composite. Examination 6 months after treatment revealed periodontal health, good esthetics, and normal function.

26/131. Endodontic and periodontal treatments of a geminated mandibular first premolar.

    AIM: To describe a rare case of gemination involving a mandibular first premolar. SUMMARY: The complex morphology of geminated teeth renders their endodontic and periodontal management difficult. Root canal and periodontal treatments were performed on a geminated mandibular first premolar with three canals. Clinical examination showed two separated crowns with united roots. Radiographically, two distinct pulp chambers with two joined and a third independent canal were seen. Conventional root canal treatment resulted in complete healing of the apical lesion. However, the occurrence of a vertical fracture led to the extraction of the mesial segment. At the follow-up visit, the distal segment was clinically healthy and continued to satisfy functional demands.

27/131. Unusual fracture of a mandibular second premolar: a case report.

    Root fractures of posterior teeth, which are defined as fractures involving cementum, dentin, and pulp, are relatively uncommon among dental traumas. This study describes an unusual horizontal fracture of a mandibular second premolar. The tooth was asymptomatic and the fracture unnoticed until the crown broke off completely. The patient had no recollection of a causative event nor was there any evidence of previous physical trauma. The tooth was extracted, embedded in resin, sliced, and examined with different light microscopes. It was concluded that the tooth had been damaged previously but not to the extent that the pulp was seriously damaged. Resorption over a period of time eventually caused the final fracture.

28/131. Cvek pulpotomy: report of a case with five-year follow-up.

    Partial pulpotomy (Cvek pulpotomy) is the treatment of choice for injured permanent incisor teeth with exposed vital pulp tissue and immature apices. This treatment preserves pulpal function, thus allowing continued root development. The present report describes the case of a permanent incisor with incomplete root end closure that underwent a Cvek pulpotomy, with subsequent apical closure. Five years post pulp therapy, the tooth remained symptom free.

29/131. Repair characteristics of horizontal root fracture: a case report.

    Radicular fractures in permanent teeth are uncommon injuries among dental traumas, comprising 0.5-7% of the cases. Fracture occurs most often in the middle-third of the root and rarely at the apical-third. The present paper reports a clinical case of a horizontal radicular fracture located between the middle- and apical-third of a upper left-central incisor followed-up for over 3 years. The tooth was extracted owing to periodontal reasons. Histomorphologically, it showed pulp-vitality preservation and root healing by hard-tissue deposition.

30/131. Pathologic interactions in pulpal and periodontal tissues.

    Both endodontic and periodontal disease are caused by a mixed anaerobic infection. The pathways for the spread of bacteria between pulpal and periodontal tissues have been discussed with controversy. This article is an attempt to provide a rational approach to the perio-endo/endo-perio question based on a review of the relevant literature. In the light of evidence, clinical concepts for the diagnosis and treatment of lesions involving both periodontal and pulpal tissues are discussed.
