1/3. empyema caused by trichomonas.

    empyema is one of the potential complications of lower respiratory tract infections. Very rarely, in predisposed individuals, empyema can be caused by trichomonas species, of which trichomonas tenax appears to be the most common cause. Here, we present a case of trichomonal empyema in a 56-year-old man and review the available literature of this rare occurrence.

2/3. empyema with trichomonas species.

    A 35-year-old alcoholic male patient developed an empyema secondary to presumed aspiration pneumonia. Trichomonads were demonstrated on wet mounts and Wright-Giemsa-stained, air-dried smears of pleural fluid. Treatment with metronidazole, other antimicrobial drugs, and drainage was successful in curing the empyema.

3/3. trichomonas tenax empyema in an immunocompromised patient with advanced cancer.

    A 53-year-old male acromegalic patient with advanced rectal adenocarcinoma developed pleuritis in the course of cobalt irradiation, steroid treatment and chemotherapy. Examination of drained pleural fluid demonstrated numerous motile organisms, which were identified as trichomonas tenax by Giemsa staining. peptostreptococcus micros was also detected in the cultures of pleural fluid and blood. Treatment with metronidazole successfully eliminated the protozoa and cured the pyothorax.
