    Urethral diverticula with calculi are rare. This 5x6 cm calculus presented in a patient with recurrent urinary tract infections. Local excision was effective.

12/224. Symptomatic paraurethral corpus spongiosum cyst in a male patient.

    The case of a 25-year-old man presenting with lower urinary tract symptoms who was found to have a noninflammatory posterior corpus spongiosum cyst is presented. The case was successfully managed by surgical excision and presents an uncommon cause of lower urinary tract dysfunction through irritation of the posterior urethra.

13/224. Gracilis transposition in complicated perianal fistula and unhealed perineal wounds in Crohn's disease.

    OBJECTIVE: To assess the efficacy of transposition of gracilis muscle in the treatment of chronic recurrent fistulas and unhealed perineal wounds after proctectomy in patients with Crohn's disease. DESIGN: Retrospective study. SETTING: Academic clinic, united states. SUBJECTS: 7 patients with Crohn's disease: 3 had unhealed perineal wounds and persistent sinuses; 2 had had several attempts to repair rectovaginal fistulas; 1 had a rectourethral fistula; and 1 a pouch vaginal fistula. INTERVENTION: Transposition of the gracilis muscle. MAIN OUTCOME MEASURE: Healing. RESULTS: Mean follow up was 18 months (range 3-30). All patients operated on for unhealed perineal wounds had healed completely within 3-6 months. The patients with a rectovaginal fistula and a rectourethral fistula had both healed by 1 month postoperatively. Two fistulas recurred, and the small pouch-vaginal fistula remained but was asymptomatic. CONCLUSIONS: Transposition of the gracilis is a viable option for the treatment of persistent sinus and unhealed perineal wound after proctectomy for Crohn's disease. It could also be an option before proctectomy for patients with other types of Crohn's-related or complicated fistulas for whom other treatments have failed. A larger series will be required before a definite conclusion can be drawn.

14/224. Ulceration of the urethral meatus after simultaneous pancreas-kidney transplantation.

    A 39-year-old male developed painful ulceration of the glans penis following simultaneous pancreas and kidney transplantation for end-stage renal failure complicating insulin-dependent diabetes mellitus. Infection was excluded. Diversion of the pancreatic secretions away from the urinary bladder into the bowel resulted in healing.

16/224. Repair of traumatic urethral fistula and huge tissue defect with Lehoczky's island flap.

    A 20-years-old male patient suffered pelvic bone fracture and a penetrating urethral injury through the perineum due to a car accident. The injury and the unsuccessful reconstruction resulted in a large perineal tissue defect, urethral fistula, and dislocation of the anus close to the fistula. The authors performed a successful reconstruction; closure of the urethral fistula, sinking of the anus to its proper place, and substitution of the missing skin and subcutaneous tissue with Lehoczky's flap. The flap with its good blood supply and mass of tissue repaired the defect and promoted the rapid, functionally and cosmetically excellent result.

17/224. Rectourethral fistula and massive rectal bleeding from iodine-125 prostate brachytherapy: a case report.

    iodine-125 brachytherapy is an effective well-tolerated treatment for localized prostate cancer. Gastrointestinal complications of brachytherapy (minor rectal bleeding or tenesmus) are uncommon. Rectal ulceration or rectourethral fistulas after prostate brachytherapy are rare. We present a case of massive refractory rectal bleeding and rectourethral fistula, a complication of prostate brachytherapy never before reported. As a result of the patient's life-threatening symptoms aggressive surgical therapy was necessary.

18/224. Recurrent urethral hairball and stone in a hypospadiac: management and prevention.

    A 32-year-old perineal hypospadiac man presented with recurrent urethral hair growth, stone, and stricture with a history of multiple urethroplasties. He was treated by urethrolithotomy, internal urethrotomy, laser epilation of the hair-bearing urethral graft, closure of the fistula, and chemical depilation of the neourethral hair. A dilute solution of thioglycolate was prophylactically instilled into the neourethra at intervals of 3 months to ensure complete tricholysis and to prevent recurrent hair growth in the future.

19/224. Transient pseudohypoaldosteronism secondary to posterior urethral valves--a case report and review of the literature.

    In transient pseudohypoaldosteronism (TPHA), renal tubular resistance to aldosterone is thought to be secondary to renal disease. We report a case of TPHA caused by posterior urethral valves associated with urinary tract infection and review 62 cases previously reported. The infant presented with unspecific signs of vomiting and dehydration, so that pyloric stenosis was first suspected. Laboratory data and retroperitoneal sonography led to the diagnosis TPHA. This case illustrates that urine culture and renal ultrasonography should be performed in any infant with electrolyte disturbances to exclude infection or obstructive uropathy.

20/224. Congenital Penile Urethrocutaneous Fistula.

    Congenital Penile Urethrocutaneous Fistula (CPUF) is an extremely rare anomaly. Commonly congenital urethrocutaneous fistulae are associated with anorectal anomalies and urethral duplication. The posterior urethra is the part, which is mostly affected. To the best of our knowledge, only 11 such cases have been reported so far in the English literature. A case of CPUF with a short review of literature.
