1/8. Physical injury as a provoking factor in three patients with scleroderma.

    A 51-year-old female developed linear-like scleroderma in the left thigh following a linear wound caused by a car accident. 27 years later she also developed a typical diffuse cutaneous systemic sclerosis with extensive skin involvement and bibasilar pulmonary fibrosis. The second case is a 39-year-old female who had a history of Raynaud's phenomenon since early childhood. She developed a morphea following a burning injury of the left thigh. 17 years later she also developed a typical limited cutaneous systemic sclerosis with sclerodactyly, skin ulcers and subcutaneous calcinosis. The third case is a 43-year-old female who developed a typical morphea of the right elbow around the site of a previous local corticosteroid injection. The two remarkable points of these 3 cases are the possible role of physical injury in the provocation of localized scleroderma and in the first 2 cases the unusual later development of a systemic form of scleroderma.

2/8. One-bone forearm reconstruction using vascularized fibular graft for massive forearm soft-tissue and bone defect: case report.

    The authors report a case of recurrent malignant fibrous histiocytoma with extensive bone- and soft-tissue involvement, successfully treated with one-bone forearm reconstruction using a vascularized fibular graft and multiple tendon transfer. Twenty-four months after surgery, elbow and hand functions were maintained, and the patient had no disturbance of hand function in daily activity, although rotation of the forearm was sacrificed. No local recurrence or metastasis was noted. The procedure is a useful reconstructive option for complex tissue defects following wide excision of sarcoma of the forearm.

3/8. Ulnar palmar cutaneous nerve and hypothenar sensory conduction studies.

    The ulnar palmar cutaneous nerve (UPCN) is potentially useful to the electrodiagnostician. However, no definitive nerve conduction study techniques for the UPCN have been reported. The UPCN supplies sensory innervation to the hypothenar palm. This study describes an orthodromic sensory conduction technique for the hypothenar palm. Data were collected from 20 normal limbed patients (ages 22-58). Potentials were recorded over the ulnar nerve 10cm proximally and at the elbow. Distal latency was 2.19 /- 0.17msec, and distal amplitude was 12.7 /- 6.9uv. Additionally, a modified collision technique was used in several subjects to examine the possibility of volume conduction to more than one ulnar nerve branch. The techniques described may be useful in selected cases of distal ulnar nerve pathology. A case is reported in which injury to the superficial sensory branch of the ulnar nerve, with sparing of the UPCN was demonstrated electrodiagnostically. Further study is needed to determine if the UPCN can consistently be electrophysiologically isolated.

4/8. Posttraumatic upper extremity wound coverage utilizing the extended deep inferior epigastric flap.

    The extended deep inferior epigastric artery (DIEA) flap was used in 4 patients with traumatic mid upper extremity wounds. Although there are numerous flaps available for resurfacing upper extremity defects, including the groin, thoracoepigastric, and a host of microvascular flaps, in each of these patients a preoperative consideration necessitated a novel approach. The DIEA flap is based on an axial vascular watershed resulting in a long, thin, well-vascularized, supple skin flap, which proved extremely effective in solving the reconstructive needs of these patients. The flap is technically simple to elevate and quite versatile. In all patients, the flaps survived and healing of the wounds was achieved. The DIEA flap should be considered a valuable reconstructive alternative when faced with elbow and forearm defects.

5/8. Long-term results of primary repair of brachial plexus lesions in children.

    The results of brachial plexus reconstruction in adults are poor, despite the sophistication of the various methods used. However, the same methods used in neonates after obstetrical brachial plexus injury will give far better results because of the shorter distance, stronger potential of regeneration, and capacity of brain adaptation. Complete paralyses, associated root ruptures, and avulsions are very severe, and the end results cannot be evaluated before the end of growth. Although the end results show that the shoulder and elbow do not do as well as in upper-type lesions, the results at the level of the hand are encouraging, showing 75% with useful function after 8 years, even in patients with avulsion injuries of the lower roots. These results demonstrate the value of the early exploration and repair of the obstetric plexus. We also review our series of traumatic brachial plexus palsy in children. The results are poorer than in neonates, where may be related, in part, to a greater percentage of associated lesions in these patients.

6/8. Heterotopic ossification.

    Heterotopic ossification is becoming increasingly recognized as a phenomenon that can complicate trauma to the head and spinal cord. It can be a disabling accompaniment of thermal injury, and it may seriously compromise results in hip arthroplasty and the treatment of acetabular fractures. Etiologic factors, which are imprecise and incompletely understood, vary with the clinical situation. The five cases reported here illustrate the radiologic appearances, complications, and diagnostic problems, including the difficulty in determining the timing of surgical resection. Reported for the first time are cases of neurogenic heterotopic ossification associated with bilateral shoulder involvement and bilateral ulnar nerve entrapment at the elbow. Heterotopic ossification may mimic acute arthritis.

8/8. Prevention and treatment of elbow injuries in golf.

    golf interest is on the increase, resulting in a corresponding increase in golf-related tendon injuries. Injuries are best avoided by a good strength and flexibility program, proper warm-up exercises, and good stroke mechanics. Treatment is directed toward restoring the integrity of the muscle/tendon unit at the elbow and preventing recurrences.
