What are the clinical syndromes associated with opportunistic mycoses?

Dear Nick,
Opportunistic mycoses are infections due to fungi with low inherent virulence which means that these pathogens constitute an almost limitless number of fungi. These organisms are common in all environments.
The fungi most frequently isolated from immunocompromised patients are saprophytic (i.e. from the environment) or endogenous (a commensal). The most common species are Candida species, Aspergillus, and zygomycetes.
When a fungus is isolated from an immunocompromised patient, the attending physician has to distinguish between:
Colonization (which is of no major concern)
Transient fungemia (often involving C. albicans)
Systemic infection.


In immunosuppressed patients, common fungal infections may have an unusual presentation because of:
1. Atypical signs and lesions.
Malassezia furfur usually causes a rather benign and self-limited disease in normal hosts (Tinea versicolor), but in immunocompromised patients may show a rash with disseminated disease and sepsis. This organism requires long-chain fatty acids for growth. Patients receiving parenteral fat emulsions for nutrition become a walking petri plate.
2. Unusual Organ affinity.
Candida may invade liver, heart valves; Oral thrush occurs in people who are relatively immunocompetent while esophageal candidiasis occurs in those patients who are immunologically compromised. Cryptococcus may cause pulmonary and cutaneous infections.
3. Infections with systemic dimorphic fungi occurring outside endemic areas. These factors complicate the diagnosis and management of these diseases.
4. Unusual Histopathology.
Even the inflammatory reaction may be different in biopsy specimens. The normal host reaction to fungal invasion is usually pyogenic or granulomatous. In the immunodeficient host, the reaction is necrotic.

I'm in the end of my sophomore year in high school, and i've been thinking alot about a career. My therapist tells me i have already begun to psychoanalyze people, and i like to read about treatments and narcotics and the above. What field should i enter?

I can't decide if my interests would be better satisfied in a medical career, or if i should go for psychiatry? Help and i'll love you!

Whats the difference between Long QT and Short QT Syndromes?

As far as clinical significance is concerned, the difference is marginal. If the duration of QT is short, it is short QT syndrome and vice versa with the long QT version. An overview:

The "QT" interval is found on an electrocardiogram (ECG). The QT represents the electrical activity of the heart. It is measured from the initiation of the QRS complex to the end of the T-wave.

The duration of the QT-interval in normal people at a heart rate of 60 beats per minute is between 350 and 440 milliseconds.

# Short QT Syndrome: Short QT Syndrome (SQTS) is a congenital abnormality in the electrical activity within the muscle cells of the heart. SQTS is named because of it diagnostic characteristics on the electrocardiogram (ECG). In patients with SQTS the duration of the QT interval has been between 210 and 340 milliseconds.

The short QT-interval found in SQTS increases the risk of an arrhythmia originating in either the small chambers of the heart (atria) or in the big chambers of the heart (ventricles). The two arrhythmias of concern in patients with Short QT Syndrome are atrial fibrillation and ventricular fibrillation.

# Long QT syndrome (LQTS): is a congenital disorder characterized by a prolongation of the QT interval on ECG and a propensity to ventricular tachyarrhythmias, which may lead to syncope, cardiac arrest, or sudden death.

Arrhythmias also can develop for no known reason in people who have LQTS. Not everyone who has LQTS develops dangerous heart rhythms. However, if one does occur, it may be fatal.

Acquired, or noninherited, LQTS may be brought on by certain medicines or other medical conditions.  (+ info)

im 13 and im really small and weak and an skinny and i want to get abs and grow muscles so how many situps should i do to get like usher abs and how many push ups to get strong muscles

It depends on your current strength and conditioning. When your excercising, you'll want to go until you start to hurt, and then a little bit further. Don't go so far that you'll permanently hurt youself, but just far enough that you'll ache for a little while. =) No pain, no gain.  (+ info)

Has anyone smelled the perfume by Usher?

I ask because I haven't and I seen it at tjmaxx for 30 bucks and I wanted it but I can't open the box to smell it and 30 is a steep price for an unknown perfume!
It is sold at Macy's and at tjmaxx and marshalls department stores.

Why there are so many weird diseases and syndromes in humans but not in animals?

Chimps share like 98% of our DNA right? but you dont see a monkey with Down syndrome or asma, are they better biologicaly?

There are probably millions more disorders in animals, but we can't communicate like them and develop a sense of normalcy in animals. Also, keep in mind that humans are some of the most complex animals on the planet (genetically).  (+ info)
